194例根尖周外科病例的生存分析

目的:评估194例根尖周外科病例的生存功能。方法:1991—2001年实行根尖周手术的患牙共194个被回访,在一套严格的标准下进行临床和X线检查。用Kap lan-M e ier方法和Log rank test来评估生存时间。用Cox回归分析混杂因素进行分析。结果:到回访时为止,154个初次手术牙齿的生存时间中位数是92.1个月[95%可信限区间(confidence interval,C.I.)=40.9,143.4],40个再次手术的牙齿生存时间中位数是39.1个月(95%C.I.=6.1,72.1);两组之间的生存时间具有显著性差异(P<0.05)。初次手术的牙齿,术前牙槽骨的边缘丧失高度和术者对生存时间具有显著性影响(P<0.05)。结论:根尖周手术的生存随时间延长呈非线性下降,术前牙槽骨的边缘丧失高度、术者和再手术是根尖手术生存的重要影响因素。

Assessment of survival function of 194 endodontic surgery

AIM:To assess the survival function of 194 endodontic surgery.METHODS:194 surgically treated teeth between 1991 and 2001 were recalled and examined clinically and radiographically using a set of strict criteria.The Kaplan-Meier method and log rank test were used to evaluate the survival time.Confounded factors were examined by Cox regression analysis.RESULTS:The median survival time of the 154 first-time surgically treated teeth was 92.1 months(95% C.I.=40.9,143.4) and that of the 40 re-surgical cases was 39.1 months(95% C.I.=(6.1,72.1)) up to the date of recall.There was a significant difference in the survival time between the two groups.For cases after first-time surgery,the pre-operative marginal bone loss and the operator had significant influence on the survival time(P<0.05).CONCLUSION:The survival time of surgical endodontic treatment declined non-linearly with time.The preoperative marginal bone loss,operator and re-surgery were important factors affecting the survival of this treatment modality.
