医用臭氧与透明质酸钠治疗兔膝骨性关节炎的疗效比较

摘    要:目的通过建立兔膝骨性关节炎动物模型,探讨医用臭氧(O3)治疗膝骨性关节炎的可行性。方法将18只家兔用伸直位固定左后肢膝关节制成兔膝骨性关节炎模型。造模成功后测量各只兔膝关节活动度,并做磁共振检查。之后将家兔随机分为3组(n=6):对照组(骨性关节炎组),透明质酸钠组(SH组),臭氧(O3)组。O3组向左膝关节腔内注入10ml 35μg/ml的O3-O2混合气体;SH组向左膝关节腔内注射1%浓度的SH1ml,1次/周;对照组动物不做特殊处理。5周后再次做双膝关节活动度检查和MR检查,处死动物取膝关节股骨髁部位的关节软骨做病理切片,HE染色观察关节软骨的一般形态、番红O染色行Mankin’s评分。结果关节活动度检查、磁共振检查与病理检查均显示解除制动后骨性关节炎继续发展,关节腔内注射O3或透明质酸钠均可延缓关节炎的发展,两种治疗方法疗效差异无统计学意义。结论医用O3可减轻炎症,延缓软骨退化,改善关节活动功能,对骨性关节炎有治疗作用。

Abstract:Objective To investigate the feasibility of the medical ozone (O3) in treatment of knee osteoartbritis, by es tahlishing animal model of rabbit knee osteoarthritis. Methods Eighteen rabbit models of knee osteoarthritis were established by fixing their left hindIimbs with unbended position. Then the knee joint activity was measured and magnetic resonance inspection of each case was performed. The cases were randomly divided into 3 groups (each n=6): the O3 group, the sodium hyaluronate (SH) group and the control group. 10 ml O3--O2 (35μg/ml) was injected into intra articular cavity of left knees of O3 group; 1 ml SH (1 %) was injected into intra-articular cavity of left knees once a week; while control group was given no special handling. After five weeks, the knee joint activity and magnetic resonance inspection of all cases were examined again. Then all cases were executed and the articular cartilages of the femoral condyle spot of knee joint were taken to make pathological section. General shape of articular cartilages with the HE dyeing was observed and each case was recorded according to Mankin's index by safranine O dyeing. Results Joint activity inspection, the magnetic resonance inspection and the pathology inspection demonstrate that the osseous arthritis continued developing after relieving of brake; intra-articular injection with the ozone or the sodium hyaluric could delay the development of arthritis; the therapeutic effect of two methods has no statistical significance. Conclusion The medical ozone could reduce the inflammation, delay the degeneration of cartilage, improve the joint activity function and have the treatment function to the osseous arthritis.
