中心型肺癌侵犯肺静脉CT表现与手术对照分析

目的：探讨中心型肺癌侵犯肺静脉的ＣＴ表现与检查价值。方法：３０例患者均有肺静脉受累，且经手术病理证实，术前均行ＣＴ平扫及增强扫描。对比分析肺静脉受累的ＣＴ征象和手术所见。结果：手术所见９例肿瘤与肺静脉一侧壁粘连，病理观察肿瘤只侵及血管外膜。２１例肿瘤环绕肺静脉生长，病理见１１例肿瘤已浸润血管管壁全层。ＣＴ表现。肿块与血管一侧壁分界不清或肿块与血管混为一体。结论：中心型肺癌ＣＴ检查对判断肺静脉侵犯程度有一定限度。ＭＲＩ因血液流空效应的特点，对判断血管侵犯程度有重要意义。

Central Bronchial Carcinoma:CT Features and Operational Comparison of Pulmonany Vein in Volved

Objective:To evaluale CT features and CT examicnation value of pulmonary vein invaded by central pulmonary carcinoma.Methods:30 cases with central bronchial carcinoma confirmed by operation-pathology were imaged by plain CT seans and contrast-enhanced CT scans before operation.The pulmonary veins were invaded by tumors in all cases.The CT features and operative finding of pulmonary veins involved were analysed.Results:The tumors were adhered to one side of pulmonary vein in 9 cases pathologically the adventitia of vein was invaded by the neoplastic tissue.The tumors grew around the pulmonary vein,pathologically all layers of vessel wall were invaded in anothaer 21 cases.CT features:the masses were not seperative from one side of vessel wall or the tumors together with pulmonary vein merged into an organic whole.Conclusion:CT examination is limited for determining the invading extent of pulmoary vein by pulmonary carcinoma.while,MRI may have important significence for judging the extent of invaded blood sessels due to its flow voit effect of vessel.
