甲胎蛋白异质体在原发性肝癌诊断中的价值

目的探讨甲胎蛋白异质体（AFP-L3）在诊断原发性肝癌中的临床应用价值。方法收集137例甲胎蛋白阳性患者（〉10ng/mL）血清样本,采用亲和吸附法测定AFP-L3含量,计算AFP-L3占总AFP的比例（%）。结果 AFP-L3比值在肝癌组为17.63±5.78,显著高于肝硬化组（8.24±2.49）、肝炎组（7.12±2.01）和对照组（5.49±1.78）,差异具有统计学意义（P〈0.01）;AFP-L3比值诊断原发性肝癌的ROC曲线下面积分别为0.894。结论 AFP-L3比值可以作为原发性肝癌临床诊断的分子标志物,对临床具有辅助诊断和鉴别诊断价值。

Value of Alpha-fetoprotein-L3 in the Diagnosis of Primary Hepatic Carcinoma

Objective To explore the clinical values of alpha-fetoprotein-L3 （AFP- liver cancer. Methods The serum samples of 137 patients with elevated AFP level L3 ） in diagnosis of primary （ 〉 10ng/mL） were detected by chemiluminescence immunoassay for AFP and centrifugal column method was used for AFP-L3 measure- ment. The proportion of AFP- L3/AFP （ % ） was calculated in the following analyses. Results T serum level of AFP-L3 in primary liver cancer group, liver cirrhosis group, chronic hepatitis B group, and control group were 17.63 ±5.78%, 8.24 ±2.49%, 7.12± 2.01% and 5.49 ± 1.78% ,respectively. The positive rate of AFP- L3 was significant higher in primary liver cancer group than other groups （ P 〈 0.01 ）. Area under the ROC curves of AFP- L3 was 0.894. Conclusion AFP- L3 could be used as biomarkers in the diagnosis of primary liver cancer, and may also has a clinical value in the auxiliary diagnosis and differential diagnosis.
