维持性透析患者牙周炎与微炎症状态关系初探

目的初步探讨牙周炎与维持性透析患者微炎症状态的关系以及牙周治疗在患有牙周炎的维持性透析患者治疗中的临床意义。方法选择患有牙周炎的维持性透析患者48例作为阳性组,非牙周炎的维持性透析患者38例作为阴性组,比较两组血清C反应蛋白(CRP)、血清前白蛋白(PA)水平及血液血红蛋白(Hb)浓度的差别,以及阳性组经系统牙周治疗后上述指标的变化情况。结果治疗前阳性组血清CRP较阴性组高(P<0.01),而血清PA水平及Hb水平均较阴性组低(P<0.05);系统牙周治疗后CRP显著降低(P<0.05),而血清PA水平及血液Hb水平有不同程度上升,但治疗前后Hb水平差异无统计学意义(P>0.05)。结论牙周炎是引起部分维持性透析患者微炎症状态的促进因素之一,系统牙周治疗有可能减轻患者的微炎症状态,从而降低营养不良及贫血等并发症发生的几率。

Relationship of Periodontitis and Microinflammation in Chronic Renal Failure Patients Undergoing Regular Dialysis

Objective To investigate the relationship between Periodontitis and microinflammation state in end stage renal disease patients undergoing chronic dialysis,and further to evaluate the effectiveness of systematic periodontal treatment in these patients. Methods The levels of sera proalbubin (PA), sera C-reactive protein (CRP) and Hemoglobin (HB) in 86 patients were determined respectively. Then 48 patients with periodontitis received systematic periodontal treatments which lasted for 1 year. CRP, PA and HB were examined again in 48 patients who finished the treatments. Results Higher level of CRP and lower level of PA and HB were detected in priodontitis patients, when compared with those in patients without periodontitis (P<0.05). The level of CRP decreased significantly after systematic periodontal treatment (P<0.05), and levels of PA and HB increase in some degree. Conclusion Periodontitis is one of the important reasons for microinflammatory state in chronic dialysis patients. Systematic periodontal treatment may be of helpful to redress chronic dialysis complications such as microinflammatory state as well as anemia and malnutrition.
