克林霉素诱导试验的必要性

摘    要:目的:探讨对大环内酯类耐药的β溶血链球菌、金黄色葡萄球菌、凝固酶阴性葡萄球菌对常用抗生素克林霉素的耐药性及可诱导克林霉素耐药的发生率。方法:mic法检测2005年1月至9月我院582株阳性菌,其中404株葡萄球菌,175株β溶血链球菌;其他阳球菌3株;双纸片法检测可诱导克林霉素耐药试验。结果:272株金黄色葡萄球菌中有248株为红霉素耐药,其中208株为持续性克林霉素耐药,余下的40株红霉素耐药克林霉素敏感(eryrclis)株中,有26株为可诱导性克林霉素耐药(mlbs)。132株凝固酶阴性葡萄球菌中,106株为红霉素耐药,其中67株为持续性耐药,余下的克林霉素体外敏感红霉素耐药的39株中,16株为可诱导性克林霉素耐药。175株β溶血链球菌中,红霉素耐药134株,持续克林霉素耐药84株,余下50株(eryrclis)中克林霉素诱导耐药21株,红霉素敏感株未见克林霉素诱导耐药。结论:临床微生物实验室须加强可诱导克林霉素耐药的检测,一旦阳性必报克林霉素耐药,以指导临床合理使用抗生素。

Abstract:Objective investigate macrolide resistant isolates of beta-hemolytic streptococcisaureus and coagulase negative staphylococcus sppwhich is susceplible to clidamicy in vitro testand evaluate the effect of clidamycinand the postiverate rate of clidamyein induce test in our hospital. Methods Use mie to detect 582 isolates rademly collected from january to septemberin our hospitaldouble antimicrobial agents detect the cindamycin induce test. Results Among the 272 s. aureus isolates ;248 isolates are resistant to erythromycin 208 isolates are constitutive resestance to clidamycin;among the 40 eryrclis isolates, 18 isolates are inducible resistance to clidamycin;among the 132 coagulase negative staphylococcus spp; 106 isolates are resitant to erythromycin;67isolates are constitutive resitaceto clidamycin ;among the 39 eryrclis isolates, 16 isolates areindueeresitace to elidamycin;amongthe175isolates of beta-hemolytic streptococci, 134 isolates are resistance to erythromycin;84 isolates are constitutive resistance to clidamycin;amongthe rest 50 eryiclisisolates,21 isolates are induce resistance to clidamycin;for all the erysclis isolates,clisdamycin induce tests are negative. Conclusion Clindamycin induction test must be done in clinical microbiology laboratoriesonce the test is postivesuch isolate is presumed to be resistant to clidamycin which is cost effective in clinicalantimiccrobial agents use.
