四川省名山县人民医院1998～2009年剖宫产率上升原因分析

目的探讨剖宫产率上升的原因以及降低剖宫产率的措施。方法对我院产科1998~2009年1689例剖宫产及剖宫产指征变化情况进行回顾性分析。结果剖宫产率逐年上升,特别是2004~2009年上升明显,与1998~2003年相比差异有统计学意义（P〈0.05）;在剖宫产率上升的原因中,难产因素为第一位,胎儿因素为第二位,后6年社会因素上升为第三位,与前6年相比差异有统计学意义（P〈0.05）,高龄珍贵儿因素由第六位上升为第四位,而围产儿死亡率和新生儿窒息率并无明显下降。结论手术指征放宽,使难产因素和胎儿因素成为剖宫产率上升的最主要原因,社会因素和高龄珍贵儿因素是近年来剖宫产率上升的重要因素。应加强医患沟通,充分告知手术风险,严格正确掌握剖宫产指征,加强产时监护,提高助产水平,有效降低剖宫产率。

Analysis of the increasing cesarean section rate in Mingshan People's Hospital of Sichuan Province from 1998 to 2009
