使用5F桡动脉鞘及造影导管经桡/尺动脉径路冠状动脉造影624例体会

目的探讨常规使用5F桡动脉鞘及造影导管经桡/尺动脉径路诊断性冠状动脉造影的可行性及安全性。方法选择我院住院行冠脉造影的连续性患者624例,其中男346例,女278例,年龄33～84岁,临床上均符合冠状动脉造影指征,无禁忌证。使用5F桡动脉专用穿刺鞘组首选穿刺右侧桡动脉,若失败再选择穿刺右尺动脉、左桡动脉或左尺动脉,分别使用5FJudkins型造影导管或左右共用型冠状动脉造影导管行冠状动脉造影。结果619例患者经桡/尺动脉径路成功完成左右冠脉的造影,5例经桡动脉冠脉造影失败而改股动脉径路成功。结论经桡/尺动脉入径使用5F桡动脉鞘及造影导管行冠状动脉造影具有成功率高、创伤小、患者极易接受等优点,值得广泛推广。

Diagnostic coronary angiography by transraoial or transfemoral approach witn 5F radial artery special-purpose puncture sheath and catheter in 624 cases

Objective To evaluate the efficacy and safety of the diagnostic coronary angiography by transradial or translemoral Approach with 5F radial artery special-purpose puncture sheath group and catheter. Methods Chooses the diagnostic coronary angiography continuous patient 624 examples, masculine 346 examples, feminine 278 examples, age 33 - 84 years old. On clinical conforms to the coronarography to refer to drafts,does not have the contraindication. Uses right flank the 5F radial artery special-purpose puncture sheath group first choice puncture the radial artery, if is defeated, chooses to puncture right ulnar artery, the left radial artery or the left ulnar artery again,uses the 5F Judkins radiography pipe separately or about uses in common the coronarography pipe to coronarography. Results About 619 example patients complete the coronary angiongraphy after the oar/ ulnar artery approach success ,5 examples change the femoral artery approach to be successful after the radial artery coronary radiography defeat. Conclusion The radial artery or ulnar artery Approach to use the 5F radial artery sheath and the radiography catheter seems to be both feasible and safe for coronary angiography with the more high success ratio, the very low risk of local complication and more comfort for patients, the patient is extremely easy to accept and so on merits, it is worth widely promoting.
