奥普力农治疗充血性心力衰竭的临床观察

目的 评价奥普力农对心力衰竭治疗的有效性与安全性.方法 将30例心力衰竭患者随机分为奥普力农组（n=14）与米力农组（n=16）,观察两组治疗前后临床心功能、血流动力学、脉冲多普勒超声心动描记术检查和治疗安全性等指标变化.结果 奥普力农和米力农均改善心功能,差异无统计学意义（P＜0.05）.用药后5min奥普力农组除右心房压、平均肺动脉楔压外,其他指标和米力农组各指标均较用药前改善,差异均有统计学意义;1h后奥普力农组上述指标及米力农组肺动脉楔压较用药后5min进一步改善,差异有统计学意义;之后药效稳定持续至用药结束后1h.奥普力农组用药后脉冲多普勒超声心动描记术检查指标均较用药前明显改善,米力农组则无明显改善;奥普力农对血压心率的影响更小.结论 奥普力农短期应用可有效缓解心力衰竭的临床症状,改善血流动力学指标,作用与米力农相当;并可对心脏结构和功能产生更有利的影响,且对血压、心率的影响更小而较安全.

Short-term Effects of Olprinone on Severe Heart Failure

Objective To evaluate the efficacy and safety of Olprinone in treatment of severe heart failure. Methods Thirty patients with severe heart failure were randomly treated with Olprinone （n=14） or Milrinone （n=16）. NYHA functional class, hemodynamics and Doppler echocardiogram were analyzed before and after treatment. Results NYHA functional class was improved by both Olprinone and Milrinone, with no difference between them. Olprinone significantly improved all the hemodynamic parameters as Milrinone did except right atrial pressure and pulmonary capillary wedge pressure at five minutes after continuous intravenous infusion. One hour later, these parameters were further improved with Olprinone and pulmonary capillary wedge pressure with Milrinone. The effects lasted for an hour after the end of infusion. Doppler echocardiographic parameters were significantly improved and blood pressure and heart rate were less influenced by administration of Olprinone instead of Milrinone. Conclusion Short-term use of Olprinone may improve clinical symptoms and hemodynamics, with more favorable effects on cardiac structure and function and less impact on blood pressure and heart rate.
