SPECT／CT骨显像对脊柱单发病灶的诊断价值

目的：探讨99Tc^m-亚甲基二磷酸盐（MDP）SPECT／CT检查在鉴别脊柱良恶性病变中的价值。方法：74例骨外原发性恶性肿瘤患者。行99Tc^m-MDPSPECT／CT检查，获得全身骨显像、断层显像、CT图像以及后两者的融合图像。由2名核医学科医生和1名放射科医生共同阅片，作出最终诊断。结果：74例脊柱单发病灶中，30例诊断为肿瘤骨转移，占总病灶数的40．5％；44例诊断为良性病变，占总病灶数的59．5％。脊柱单发病灶的良恶性不仅与病灶的位置相关，而且与病灶的形状相关。全身骨显像与同机CT互补，可以提高脊柱单发病灶诊断的准确率。结论：SPECT／CT检查对鉴别脊柱良恶性病变有重要的临床应用价值。

Evaluation of SPECT/CT scan for differential diagnosis of benign and malignant single spinal disease

Objective： To evaluate the value of SPECT/CT imaging in differentiating malignancy from benignancy in single spinal disease. Methods： There were 74 patients （male 34, female 40, mean age 55.2yrs） underwent bone scanning and SPECT/CT. The diagnosis was made by two doctors from Nuclear Medicine Department and one doctor from Radiologic Department based on interpretation of the nuclear imaging combined with CT. Results： Of the 74 lesions detected in whole body scan, 30 were malignant lesions and 44 were benign lesions. The malignancy or benignancy of the single spinal disease not only relate to the location, but also to the characteristic appearance of the lesion. Conclusions： SPECT/CT scan is clinically valuable in differentiating malignancy from benignancy in single spinal disease.
