风湿性心脏病二尖瓣狭窄闭式扩张术中心室纤颤复苏四例报告

二尖瓣狭窄闭式扩张术中发生心室纤颤是极为严重的并发症之一,若处理不当,死亡率高达37—60％文献统计],近十年来在普心手术中遇到四例,经复苏抢救均获成功。今对本文病例的发生原因、复苏方法及预防措施作分析和讨论,以供同道参考。

RESUSCITATION OF VENTRICULAR FIBRILLATION IN FOUR PATIENTS WITH RHEUMATIC HEART DISEASE DURING THE OPERATION OF CLOSED DILATATION FOR MITRAL STENOSIS

Ventricullar fibrillation (VF) is a serious complication during the o-peration of closed dilatation for mitral stenosis, the mortality of it was up to 37-60%. In the recent ten years the authors met 4 cases and all patients were sucess-fully resuscitated. VF happening in anesthesia insertion was 1 case, in left, auricle purse suture was 2 cases and in end-operation was 1 case. The causes of VF are related to the factors as follows. 1. serious mitral stenosis, 2. abnormal anesthesia, 3. electrolytic disturbance, 4. excessive stimulation of the heart. The sucess of resuscitation depends upon: 1, continuous heart massage, 2. electric cardio-version in time, 3. correction of the acidosis and electrolytic disturbance, 4. application of the pos-sitive cardiac drugs, 5. effective maintenance of the respiration and blood oxygen supply of the important organs, 6. prompt dilatation of the stenotic mitral valve, 7. appropriate management of the complication post cardiopulmonary resuscitation.
