超声对二尖瓣置换术后远期心功能不全的病因诊断

摘    要:
目的 探索超声心动图在二尖瓣置换 (MVR)术后远期心功能不全诊断中的作用。 方法 超声追踪随访 4 4例 MVR术后 3个月至 13年半 ,并根据手术的远期效果 ,分为康复组 (A组 )和心功能不全组 (B组 )。除注意人工瓣和自然瓣的病变外 ,还分析了这两组手术前后左房、左室及 EF的差异。 结果 超声显示 B组二尖瓣位单组或伴主动脉瓣位双组人工瓣异常 5例 ,其他自然瓣明显病变 11例。术后 B组的左室内径明显大于 A组 (P<0 .0 5 ) ,EF明显小于 A组 (P<0 .0 1)。超声提供的信息为 36 .4 % (4/11)心功能不全代偿期和 6 6 .7% (8/12 )失代偿期患者找到了导致心功能障碍的主要原因。 结论  MVR手术前后超声追踪扫查 ,对其术后远期心功能不全的诊断具有很大临床实用价值

Abstract:
Objective To evaluate the echocardiographic diagnosis of cardiac insufficiency(CI) long term after mitral valve replacement(MVR). Methods 44 MVR patients were followed up from 3 months to 13 5 years. All patients were studied with two|dimensional or color Doppler echocardiography before and after the operations. The patients have been divided into group A(21 cases) recovery group and group B(23 cases) cardiac dysfunction group after operations. Results 5 cases of arstifical valve and other 11 cases obvious valves lesions were found in B group. Inner diameter of left ventricle were larger than A group(P<0.05). EF in B group were decreased significantly than A group(P<0.01). 36 4% cardiac function was compensated and 66.7% displayed cardiac dysfunction detected by echocardiographic examination. Conclusion Color Doppler flow imaging is an effective method for diagnosing cardiac dysfunction long term after MVR. It could play an important role in clinical diagnosis of CI.;
