微创与开颅手术治疗高血压脑出血的效果比较(附85例报告)

目的探讨应用微创手术治疗高血压脑出血的临床疗效。方法微创组32例。术前定位根据头颅CT显示的血肿中心为靶点，应用YL-Ⅰ型一次性颅内血肿微创粉碎针（北京产）行脑内血肿穿刺抽吸，清洗，减压后接引流管持续引流治疗。开颅组30例常规开颅手术。结果微创组治愈好转25例（78．1％）死亡7例，开颅组治愈好转21例（70％）死亡11例，两组比较微创组疗效显著（P〈0．05）。结论应用微创YL-Ⅰ型一次性颅内血肿微创针治疗高血压脑出血，其效果明显优于惯用的开颅手术，且具有安全，创伤小，恢复快，费用低等优点，尤适于基层医院推广使用。

Effect Comparison between Microtraumatic Operation and Routine Craniotomy in Treating Hypertensive Intracerbral Hemorrhage

Objective To discuss the clinical effect between microtraumatic operation and routine craniotomy in treating hypertensive intracerebral hemorrhage.Methods 62 patients out of the 85 patients with hypertensive intracerebral hemorrhage hospitalized from June 2001 to December 2004 were randomly selected,and then divided into two groups:32 patients in microtraumatic group and 30 in routine craniotomy group;for the 32 patients in microtraumatic group,CT was applied first to set the central target and then,YL-I Microtraumatic Puncture Needle was used to made intracerebral hemorrhage puncture,sustained suction was given after aspiring,cleaning and decompressing;for the 30 patients in the other group,the routine craniotomy was given.Results 25 patients in the microtraumatic group recovered(78.1%),7 patients died,while 21 patients in the routine craniotomy group recoverd(70%) and 11 patients died;the effect of the microtraumatic group was much better than that of the routinecraniotomy group(P<(0.05)).Conclusions YL-I Microtraumatic Puncture Needle gives out a significant superior effect to that of routine craniotomy,it has such advantage of greater safety,microtrauma,quick recovery,low cost and so on,especially suitable to be used in small hospitals.
