经皮冠状动脉扩张及支架术后穿刺口血肿发生率与护理疗效的探讨

目的探讨降低冠状动脉扩张(PTCA)及支架术后穿刺口血肿发生率的有效护理措施。方法将1996年到2003年的246例PTCA及支架术患者(为随机取样,经分析后具有可比性)分为AB两组:A组为1996年至1999年的96例患者,对该组的患者术后只施行常规护理。B组为1999年至2003年的150例患者,对该组患者除施行常规护理外,还特别重视术前教育、术后动脉鞘管的拔除、术口的加压包扎、严密观察伤口情况及重视肝素钠的应用。将A、B两组患者术后术口血肿发生率作比较。结果A组血肿发生率为8.33%,B组血肿发生率为4.66%,B组血肿发生率较A组下降了3.67%(P<0.05)。结论加强对PTCA及支架术的护理,可大大降低该手术患者的血肿发生率。

The discussion of occurrence of haematoma on puncture and curative effect of nursing after PTCA & Bracket placement

Abstact:Objective:To discuss the effective nursing measure of reducing the occurrence rate of haematoma on puncture after PTCA (percutaneous transluminal cornary angioplasty).Methods:To divide 246 cases (which are randomly sampled and are comparable after analyzed) of PTCA &bracket placement during 1993 to 2003 into two groups:group A consisted of 96 cases during 1993 to 1999,who received only routine care,group B consisted of 150 cases during 1999 to 2003,who received routine care as well as special preoperative instruction,postoperatively arterial conduit removal,enhancing enswathement of incision,observation of incision condition and the application of heparin sodium.Then compare the occurrence rate of postoperative haematoma on puncture documented in group A with that of group B.Results:The occurrence rate of hematoma of group A is 8.33%,while that of group B is 4.6%.After statistic analysis,it is significant that the occurrence rate of hematoma of group B has been reduced by 3.67%.(P < 0.05)compared with that of group A.Conclusion:Intensified perioperative care in the PTCA &Bracket placement is propitious to reduce the occurrence rate of hematoma.
