气管内冲洗联合NCPAP治疗新生儿肺炎呼吸衰竭26例分析

摘    要:目的观察气管内冲洗联合NCPAP治疗新生儿肺炎呼吸衰竭的疗效。方法对26例新生儿肺炎呼吸衰竭予行气管内冲洗联合NCPAP治疗,并观察治疗前后呼吸、心率等临床症状及动脉血气分析的变化。结果 26例患儿除2例治疗无效在6 h内改CMV治疗外,余24例治疗前后临床症状和血气指标改善明显,差异有显著统计学意义(P<0.01)。结论气管内冲洗联合NCPAP对新生儿肺炎呼吸衰竭具有明显的治疗效果,可避免或减少机械通气的应用。

Abstract:Objective To observe the curative effects of endotracheal irrigation combined with NCPAP in the treatment of neonatal pneumonia complicated by respiratory failure. Methods Twenty-six neonates with neonatal pneumonia complicated by respiratory failure received endotracheal irrigation combined with NCPAP therapy. The changes of respiration, heart rate and other clinical symptoms and arterial blood gas analysis before and after treatment were observed. Results All of the 26 neonates significantly improved after treatment regarding to the clinical symptoms and the arterial blood gas indicators except 2 neonates turning to CMV therapy within 6 hours due to ineffective treatment, with statistically significant differences (P 〈 0.01). Conclusion Endotracheal irrigation combined with NCPAP shows significant curative effects in the treatment of neonatal pneumonia complicated by respiratory failure, and therefore the application of mechanical ventilation can be avoided or reduced.
