基底节区血肿术后血肿残留量对近期预后的影响

目的探讨高血压基底节区血肿术后血肿残留量对近期预后的影响。方法 60例高血压基底节区脑出血患者早期行小骨窗显微镜下血肿清除手术,术后48 h复查头颅CT查血肿残留量,观察不同血肿残留量术后48 h的意识改善程度和术后1个月患侧肌力恢复程度,并作统计学处理。结果术后48 h GCS均值比较:血肿残留量<5 mL组与5～10 mL组无显著性差异(t=1.867,P>0.05),血肿残留量5～10 mL组与>10 mL组有显著性差异(t=2.917,P<0.01);术后1个月肌力均值比较:血肿残留量<5 mL组与5～10 mL组有显著性差异(t=2.793,P<0.01),血肿残留量5～10 mL组与>10 mL组有显著性差异(t=2.234,P<0.05)。结论基底节区HICH术中血肿清除越彻底,术后血肿残留量越少,术后近期疗效越好。

The influence of hematoma residues on early prognosis of hypertensive basal ganglia cerebral hemorrhage

Objective To investigate hematoma residues influencing on early prognosis of hypertensive basal ganglia cere- bral hemorrhage. Methods A total of 60 patients with hypertensive basal ganglia cerebral hemorrhage were treated by mi- crosurgery with small bone window early stage, reviewed head CT at 48h after operation, to statistical analysis GCS at 48h after operation and muscle strength at lm after operation in different residual hematoma. Results Forty-eight h GCS mean in comparison： hematoma residues 〈5 mL group with 5-10 mL was no significant difference （t = 1.867, P 〉 0.05）, hematoma residues 5-10 mL group with 〉I0 mL group was significant differences （t = 2.917, P 〈 0.01 ）; the mean in mus- cle strength after lm comparison：hematoma residues 〈5 mL group with 5-10 mL group was significant difference （t = 2.793, P 〈 0.01 ）, ＇hematoma residues 5-10 mL group with 〉10 mL group was significant difference （t = 2.234, P 〈 0.05）. Conclusion Residual hematoma has significant impact on early prognosis of hypertensive basal ganglia cerebral hemor- rhage, less residual postoperative hematoma, the better early prognosis.
