腹腔镜联合药物治疗卵巢子宫内膜异位症的临床观察

目的观察腹腔镜手术联合孕三烯酮药物治疗卵巢子宫内膜异位症的临床效果及安全性评价。方法选择2009年1月~2010年12月在本院行腹腔镜手术治疗并术后诊断为卵巢子宫内膜异位症患者106例,随机分为治疗组及观察组,治疗组患者于术后第1次月经来潮第2天即给予疗程6个月的孕三烯酮口服,观察组术后不予任何药物治疗,所有患者均完成随访,末次随访日期至2011年12月,观察比较两组治疗效果及复发情况。结果两组患者腹腔镜手术均顺利完成,全部完成随访,治疗组用药不良反应轻微,均完成足疗程治疗,且治疗有效率及术后妊娠率明显高于对照组,复发率明显低于对照组,差异有统计学意义(P<0.05)。结论卵巢子宫内膜异位症患者采用腹腔镜手术,术后联合孕三烯酮药物治疗,治疗效果显著,显著降低复发率,怀孕率得以提高,副反应小,值得临床推广应用。

Effects of laparoscopic surgery combined with medication in treatment of o- varian endometriosis

Objective To investigate the effects and safety of combinative treatment with laparoscopy and gestrinone on o- varian endometriosis. Methods A total of 106 cases diagnosed ovarian endometriosis were randomly divided into two groups,the treatment group and the observation group by laparoscopy during form January 2009 to December 2010.The pa- tients in the treatment group were given 2.5 mg of gestrinone for 6 months after operation. The cases in observation group had not taken any hormone. All patients were followed up until December 2011 to observe the therapeutic effect and re- currence. Results All the patients were successfully completed the laparoscopic surgery, the treatment group drug adverse reaction mild, all patients finished the total course, and treatment efficient and postoperative pregnancy rate was signifi- cantly higher than the observation group, the recurrence rate was significantly lower than the control group, the difference was statistically significant （P 〈 0.05）. Conclusion Ovarian endometriosis patients using laparoscopic surgery, postoperative joint pregnant three ketene drug treatment, the treatment effect is remarkable,and can remarkablely decrease the recurrence rate, the pregnancy rate can improved, and have small side effects, worth of clinical application.
