自凝刀射频消融术联合米非司酮治疗子宫肌瘤的疗效观察

目的分析超声引导下自凝刀射频消融术联合米非司酮治疗子宫肌瘤的临床疗效。方法收集我院2010年1月～2012年9月收治的子宫肌瘤患者112例。按随机数字表法分为观察组58例,给予自凝刀手术治疗,同时联用米非司酮12.5 mg/次.d,连续3个月;对照组54例给予单独的自凝刀射频消融术。比较两组临床有效率;促卵泡激素(FSH)、孕酮(P)、黄体生成素(LH)、睾酮(T)、雌二醇(E2)水平改善;血液红细胞浓度、血红蛋白浓度、红细胞压积水平改善。结果对照组总有效率为87.0%(47/54),观察组总有效率为98.3%(57/58),两组比较P<0.05。治疗后两组FSH、LH、E2、P水平均显著变化,差异具有统计学意义(P<0.05)。T水平无显著变化,在统计学上不具有显著性(P>0.05)。治疗后观察组血液红细胞浓度、血红蛋白浓度、红细胞压积水平均显著高于对照组,在统计学上具有显著性(P<0.05)。结论射频消融术与米非司酮联合治疗子宫肌瘤可明显提高治愈率,值得临床推广应用。

Observation of efficacy of self-clotting cutter radiofrequency ablation com- bined with mifepristone in treatment of uterine fibroids

Objective To analyze the efficacy of ultrasound-guided self-clotting cutter radiofrequency ablation combined with mifepristone in the treatment of uterine fibroids. Methods All of 112 patients with uterine fibroids admitted to and treated in our hospital from January 2010 to September 2012 were collected and randomly divided into the observation group and the control group according to the random number table. Fifty-eight patients of the observation group were given self-clotting cutter surgical treatment and mifepristone 12.5 rag/time/day at the same time for 3 continuous days. Fifty-four patients of the control group were given only self-clotting cutter radioffequeney ablation. The clinical effective rate, the im- provement of follicle stimulating hormone （FSH）, progesterone （P）, luteinizing hormone （LH）, testosterone （T） and estradiol （E2） levels, and the improvement of red blood cell concentration, hemoglobin concentration, hematocrit levels of the two groups were compared. Results The total effective rate of the control group was 87.0% （47/54） and the total effective rate of the observation group was 98.3% （57/58）, with significant difference between the two groups （P 〈 0.05）. After the treat- ment, the FSH, LH, E2 and P levels of both groups manifested significant changes, with statistically significant difference （P 〈 0.05）. But the T level did not change significantly, without statistically significant difference （P 〉 0.05）. After the treatment, the observation group was significantly higher than the control group in the red blood cell concentration, hemoglobin concentration and hematocrit levels, with statistical significance （P 〈 0.05）. Conclusion In the treatment of u- terine fibroids, radiofrequency ablation combined with mifepristone can significantly improve the cure rate, thereby worthy of clinical application.
