双腔支气管插管救治大咯血的效果观察

目的观察用双腔支气管插管救治大咯血的效果。方法选择2010年4月30日～2012年4月30日我院急诊科、呼吸科、胸外科和中心ICU收治的大咯血患者45例,随机分为对照组22例和研究组23例。对照组接受普通气管插管救治;研究组接受双腔支气管插管救治。比较两组抢救成功率及插管治疗后的咯血总量。结果对照组抢救成功率36.36%明显低于研究组69.57%,差异有统计学意义(P<0.05)。对照组接受气管插管后咯血总量为(495±12.5)mL,研究组接受双腔支气管插管后咯血总量为(410±16.0)mL,两组比较差异有统计学意义(P<0.05)。结论双腔支气管插管救治大咯血有很好的效果。

Effect observation of double lumen endobronchial intubation in treatment of massive hemoptysis

Objective To observe the effect of double lumen endobronchial intubation in treatment of massive hemoptysis. Methods All of 45 patients with massive hemoptysis who were hospitalized in Emergency, Respiratory Medicine, thoracic surgery and ICU from April 30, 2010 to April 30, 2012 were randomly divided into two groups. The control group with 22 patients received common trachea vessel treatment;the research group with 23 patients received double lumen endo- bronchial intubation treatment. The success rate of rescue and the total amount of hemoptysis were compared after tracheal intubation between the two groups. Results The success rate of rescue of the contrast group was 36.36%, and the success rate of rescue of the research group was 69.57%. There was significant difference between the two groups （P 〈 0.05）. The total amount of hemoptysis of the control group after tracheal intubation was （495±12.5）mL, and the total amount of hemoptysis of the research group after tracheal intubation was （410±16.0）mL. There was significant difference between the two groups（P 〈 0.05）. Conclusion The effect of double lumen endobronchial intubation in treatment of massive hemoptysis is very good.
