人巨细胞病毒激活感染与颈动脉粥样硬化斑块不稳定性的临床研究

摘    要:目的探讨人巨细胞病毒(HCMV)激活感染与颈动脉粥样硬化的关系。方法采用免疫荧光法检测外周血自细HCMV—PP65抗原作为HCMV激活感染的指标,对212例颈动脉粥样硬化患者分别进行HCMV—PP65抗原、血清基质金属蛋白酶-9(MMP-9)、肿瘤坏死因子-α(TNF—α)水平检测和颈动脉超声检查。结果HCMV—PP65抗原阳性组、阴性组血清MMP-9含量分别为(242.25±81.03)、(159.57±79.20)pg/mL,差异有统计学意义(P〈O.01);两组TNF—α含量分别为(3.04±0.37)、(1.37±0.17)ng/mL,差异有统计学意义(P〈O.01);HCMV—PP65抗原阳性组44例(69.84%)存在不稳定性斑块,HCMV—PP65抗原阴性组72例(48.32%)存在不稳定性斑块,差异有统计学意义(x^2=8.82,P〈0.01)。结论人巨细胞病毒激活感染可引起MMP-9、TNF—α水平增高,可能与颈动脉粥样硬化斑块不稳定性有关。

Abstract:Objective To study the human cytomegalovirus activate infection concerned with carotid atherosclerosis. Methods HCMV-PP65 in peripheral blood leukocytes, which was used as an indicator of active HCMV infection, was detected by immunofluorescence. A total of 212 patients with carotid atherosclerosis was carried out to check HCMV-PP65 antigen level of MMP-9 and TNF-α and carotid ultrasonography. Results Contents of MMP-9 and TNF-α in two group difference were (242.25±81.03), (159.57±79.20) pg/mL and (3.04±0.37), (1.37±0.17)ng/mL, there were significant difference between 2 groups (P〈0.01, P〈0.01). The positive and negative of HCMV-PP65 antigenaemia with destabilization of carotid plaque were 69.84%, 48.32% respectively. Statistically, there was significant difference of HCMV-PP65 antigenaemia between the two groups (x^2=8.82, P〈0.01). Conclusion Activate infection of HCMV may induce the raising of level of MMP-9 and TNF-±α. It may be associated with the instability of carotid atherosclerosis plaque will be more.
