超声引导下射频消融联合米非司酮治疗子宫肌瘤临床评价

目的 探讨超声引导下射频消融联合米非司酮治疗子宫肌瘤的临床疗效及对单核细胞趋化因子-1(MCP-1)水平的影响。方法 选取医院2017年1月至2018年1月收治的子宫肌瘤患者138例,按抽签法分为观察组和对照组,各69例。两组患者均予超声引导下射频消融治疗,观察组患者术后加服米非司酮片。结果 观察组总有效率为98.55%,明显高于对照组的84.06%(P<0.05);治疗后,观察组患者子宫体积和子宫肌瘤体积显著小于对照组,孕酮(P)、雌二醇(E2)、卵泡刺激素(FSH)和黄体生成素(LH)水平及血管内皮生长因子(VEGF)、糖类抗原125(CA125)水平、MCP-1和胰岛素样生长因子-1(IGF-1)水平均显著低于对照组,白细胞介素2(IL-2)水平显著高于对照组(P<0.05);观察组不良反应发生率与对照组相当(5.80%比7.25%,P>0.05)。结论 超声引导下射频消融联合米非司酮治疗子宫肌瘤,可明显缩小患者的子宫体积、子宫肌瘤体积,降低性激素水平和MCP-1水平。

Ultrasound-Guided Radiofrequency Ablation Combined with Mifepristone in the Treatment of Uterine Leiomyoma

Objective To analyze the efficacy of ultrasound-guided radiofrequency combined with mifepristone in the treatment of uterine leiomyoma and its effect on monocyte chemokine-1(MCP-1).Methods A total of 138 cases of uterine leiomyoma treated from January 2017 to January 2018 in the hospital were selected and divided into the observation group and control group according to the envelope method,69 cases in each group.The control group was treated with ultrasound-guided radiofrequency therapy,and the observation group was treated with mifepristone after operation on the basis of the control group.Results The total effective rate of the observation group was 98.55%,which was significantly higher than 84.06%of the control group(P<0.05).The uterine volume and uterine myoma volume in the observation group were significantly lower than those in the control group after treatment(P<0.05).After treatment,the uterine volume and uterine fibroid volume in the observation group were significantly smaller than those in the control group,and the levels of progesterone(P),estradiol(E2),follicle stimulating hormone(FSH),luteinizing hormone(LH),vascular endothelial growth factor(VEGF),carbohydrate antigen 125(CA125),CPC-1,and insulin-like growth factor-1(IGF-1)were significantly lower than those in the control group;the level of interleukin-2(IL-2)was significantly higher than that in the control group(P<0.05).The incidence of adverse reactions in the observation group was similar to that in the control group(5.80%vs.7.25%,P>0.05).Conclusion Ultrasound-guided radiofrequency combined with mifepristone in the treatment of uterine leiomyoma has a significant effect,which can significantly reduce the uterine volume,uterine leiomyoma volume,sex hormone level and MCP-1 level.
