经乳晕入路腔镜与开放手术治疗甲状腺良性肿瘤的对比研究

目的:探讨腔镜甲状腺切除术的可行性及安全性。方法:2002年5月至2008年6月收治甲状腺良性肿瘤患者155例,按患者意愿及手术适应证分为两组,45例行经乳晕入路的腔镜手术,110例行开放手术,比较两组手术时间、出血量、住院时间、住院费用等。结果:腔镜组45例均顺利完成手术,平均手术时间(116±16.7)min,平均术中出血(40.0±23.6)ml,无中转开放手术,术后2～3d拔除引流管,无声嘶或甲状旁腺损伤发生,平均住院(7±3.5)d,术后随访3～14个月,无复发病例,美容效果满意。结论:腔镜甲状腺手术安全、有效、可行。

The comparative study of endoscopic surgery via the areola of breast approach and open surgery for benign thyroid tumors

Objective:To discuss the feasibility and security of endoscopic thyroidectomy.Methods:From May 2002 to Jun.2008,one hundred and forty-five patients with benign thyroid neoplasms were divided into two groups according to patients' will and surgical indications,45 of them underwent endoscopic thyroidectomy via a breast approach,and the other 110 patients received open surgery.Clinical data of the two groups such as operation time,blood loss,hospital stay and the cost were compared.Results:Forty-five endoscopic operations were successfully completed,with an average operative time of (116±16.7)min,and the average blood loss was (40.0±23.6)ml.No case was converted to open surgery.Drainage tubes were removed 2-3 days after operation.No hoarseness or parathyroid injury occurred,and the average hospital stay was (7±3.5)d.No recurrence was found during the follow-up of 3 to 14 months,and cosmetic results of all cases were satisfactory.Conclusions:Endoscopic thyroidectomy is safe,effective and feasible.
