Study on the MR fat suppression technique in assessing the imaging effect of sacroiliac joint of ankylosing spondylitis

Abstract:Objective:To analyze the imaging effect of fat suppression technique of magnetic resonance imaging(MRI)in assessing sacroiliac joint of ankylosing spondylitis(AS).Methods:98 AS patients who admitted to hospital were selected,and all patients underwent MRI examination,and the fat suppression(FS)sequence,short time inversion recovery(STIR)sequence and spectral attenuated inversion recovery(SPAIR)sequence were adopted.The image homogeneity of left and right sides of sacroiliac joint,the signal to noise ratio(SNR)and contrast noise ratio(CNR)of fat and muscle at hip,and those of bone marrow edema(BME)among three kinds of fat suppression sequences of patients were compared.The relevant scores of image quality of three kinds of fat suppression sequences were analyzed.And the size of lesion and morphological comparison were showed by the scans of FS sequence and STIR sequence.Results:The image homogeneities of sacroiliac joint of right side were significantly higher than those of left side in FS sequence and SPAIR sequence(t=1872.28,t=273.00,P<0.05).The image homogeneities of left and right sides of sacroiliac joint in SPAIR sequence were significantly higher than those of FS sequence and STIR sequence(F=76142.96,F=42911.44,P<0.05),respectively.There were significant difference in SNR and CNR of fat and muscle of FS and SPAIR sequences between left and right sides(t=16.206,t=15.483,t=5.178,t=3.632,P<0.05).The scans of three kinds of fat suppression sequences of 98 patients indicated that the BME of 59 patients were obvious,and the SNR and CNR of BME in STIR sequence were significantly higher than those in FS sequence and SPAIR sequence(F=29.41,F=30.00,P<0.05),respectively,and the display of STIR sequence was best on BME.STIR sequence was significantly superior to FS sequence and SPAIR sequence in fat suppression homogeneity,the significance of lesion,artifact and overall quality(F=1972.62,F=2678.60,F=882.61,F=573.01,P<0.05),respectively,and FS sequence was similar to SPAIR sequence.There was no difference in the detection rate between FS sequence and STIR sequence in the same type of lesions,but the size and morphology of lesions in the display of oblique coronal scan of STIR sequence was better than those of oblique axial scan of FS sequence.Conclusion:STIR sequence of MRI fat suppression technique can be used in the examination of sacroiliac joint of AS patients,and its oblique coronal scan can clearly show the size and shape of lesions,and its clinical application is better than that of FS and SPAIR sequences.
