X线平片及CT在股骨颈疝窝诊断中的应用

目的分析股骨颈疝窝的X线及CT表现并比较二者对本病的敏感性及特异性。方法回顾性分析我院540例骨盆、髋关节平片及髋关节CT发现的26例股骨颈疝窝的影像学资料。结果 20例X线平片上表现为股骨颈中轴线或其外上方的类圆形伴有硬化边的透亮区,22例(含16例平片异常者)CT均表现为股骨颈前方皮质下类圆形骨质缺损,大部分可见与病灶相通的局部皮质裂隙状缺损,内为较均匀的软组织密度灶(或)液体密度,边缘清楚伴有硬化缘,其中1例X线平片表现不明显。结论 CT能显示股骨颈疝窝的内部细节,大部分病灶具有特征性表现,比X线具有更高的敏感性及特异性。

Application of X-ray and CT in the Femoral Neck Diagnosis

Objective Analyze the characteristics of X-ray and CT for femoral neck and compare their sensitivity and specificity for illnesses.Methods A retrosp-ective analysis of all imaging data of radiographs about 540 cases of pelvic and hip and the information about 26 cases,a total of 28 femoral neck found by CT hip.Results For X ray of 20 cases,there are transparent areas on the femoral neck axis or on the hardened outer edge.For 22 cases（including 16 cases of flat sheet abnormality） CT shows that there is circular bone defect in the front of femoral neck and that crashed-like defect for focal cortical connected to lesion is visible,inside which is uniform density of soft tissue lesions or liquid density,clear margin with sclerotic margin.Among them,X ray manifestations for 1 case is not clear.Conclusion CT has higher detection rate and can display interior details of the femoral neck where most lesions are characterized.Compared with X-ray,CT has higher sensitivity and specificity.
