膈下逐瘀汤加减治疗腹部术后并发膈下脓肿的临床观察

目的：观察膈下逐瘀汤治疗术后并发膈下脓肿的临床效果。方法：随机将90例术后并发膈下脓肿的患者分为治疗组和对照组,两组患者均采用抗生素抗感染、营养支持、B超引导下腹腔穿刺抽脓、置引流管持续引流等综合治疗。治疗组辅以膈下逐瘀汤,对照组只用抗生素治疗而不加中药。结果：1个疗程后,治疗组治愈率为75.56%,总有效率95.56%;对照组治愈率为51.11%,总有效率84.44%;两组治愈率、再手术率经χ2检验,差异有统计学意义（P〈0.05）,而总有效率差异无统计学意义（P〉0.05）。结论：膈下逐瘀汤辅助治疗腹部术后膈下脓肿可明显改善症状,缩短疗程,优于单纯西医治疗,可在临床推广应用。

Clinical observation on treatment of subphrenic abscess after abdominal operation with medication of Gexiazhuyutang

Objective To investigate the clinical effect of Gexiazhuyutang on subphrenic abscess after abdominal operation.Method Total 90 cases of subphrenic abscess after abdominal operation were randomly divided into the treatment group and the control group.Patients in both groups were treated with synthetic therapy including antibiotic use,nutritional support,ultrasound-guided abdominal puncture,placing peritoneal cavity drainage tube for continuous drainage and so on.All patients in the treatment group were treated with the medication of Gexiazhuyutang as an adjuvant therapy,and those in the control group did't receive TCM therapy.Results After one course of treatment,the cured rate was 75.56% in the treatment group and 51.11% in the control groups,the total effective rate in the treatment group and the control group were 95.56% and 84.44%,respectively.There were significant differences in the cured rate and reoperation rate between the two groups by Chi-square test(P<0.05),and there was no statistical significance in total effective rate.Conclusion Medication of Gexiazhuyutang for subphrenic abscess after abdominal operation as an adjuvant therapy could improve symptoms and shorten the course of treatment,and the treatment is better than Western medicine and could be applied for clinic.
