韦格内肉芽肿和显微镜下多血管炎临床诊断分析

目的：探讨韦格内肉芽肿(Wegener’s granulomatosis，WG)和显微镜下多血管炎(microscopic polyangiitis，MPA)临床特征、病理学和免疫学检查的诊断价值。方法：回顾性分析典型病例。结果：WG和MPA可侵犯多个脏器，其中肺、肾最常见。临床表现主要有发热、咳嗽、咯血、血尿。X线表现以肺部多发性结节或片状影常见。病理学检查为确诊的金标准，抗中性粒细胞胞质抗体(antineutrophil cytoplasmic antibodies，ANCA)试验可协助诊断。结论：综合患临床特征、病理学和免疫学检查结果，有助于尽早确定WG和MPA的诊断。

Diagnostic analysis of Wegener's granulomatosis and microscopic polyangiitis

Objective To explore the role of clinical feature,pathological and im mu nological examination in the di-agnosis of Wegener's granulomatosis(WG)and micr oscopic polyangiitis(MPA).Methods Retrospective analysis of typical cases was us ed.Results Multiple organs were involved in patients with WG and MPA,and the lun g and the kidney were involved most frequently.Main clinical symptoms in the pat ients were fever,cough,haemoptysis and hematuria.Multiple nodular and patched sh adow were most common in X-ray examination.Pathological examination was the gol d standard of diagnosis,and antineutrophil cytoplasmic antibody(ANCA)test was an important adjunct to pathological examination.Conclusion Combination of clinica l feature,pathological and immunological examination is helpful for the early di agno-sis of WG and MPA.
