早发型重度子痫前期保守治疗的临床探讨

目的:探讨早发型重度子痫前期的临床治疗。方法:回顾分析浙江省嘉兴市妇幼保健院2003年1月-2005年12月收治的251例重度子痫前期患者(其中早发型64例,即24-33+6天孕周发病者;晚发型187例,即≥34孕周发病者)及其围生儿297例的临床资料。分析指标包括一般情况、并发症及母婴结局。结果:早发型重度子痫前期患者分娩孕周较晚发型早(P<0.01),治疗时间较晚发型长(P<0.05),其临床症状及并发症较晚发型严重,母婴结局明显较晚发型差。结论:早发型重度子痫前期病情严重,围生儿预后不佳,应根据母胎情况,严格选择病例进行保守治疗,同时密切监测母胎病情变化。

Clinical study of conservative management of early -onset severe preeclampsia

Objective:To investigate the clinical therapy of early-onset severe preeclampsia.Methods:Retrospectively analyzed 251 severe preeclampsia mothers and their 297 neonates who were cured in Maternity and Health Hospital of Jiaxing during January of 2003 to December of 2005.The 251 severe preeclampsia patients included 64 early onsets(onset during 24 to 34 gestational weeks) and 187 late-onsets(onset after 34 gestational weeks).The general condition of the mother,complications and outcomes of both mothers and infants were surveyed.Results:The time of gestation termination in the cases with early-onset severe preeclampsia was earlier than that with late-onset type(P<0.01);in addition,the time period of treatment was also longer(P<0.05).The clinical symptoms and complications of early-onset severe preeclampsia were more severe and the outcomes of mothers and infants were significantly worse than those in late-onset ones.Conclusion:The pathogenetic condition of early-onset severe preeclampsia is severe.The outcomes of mother and infant are not good.Obstetricians should select cases strictly for expectant treatment and close monitoring is very important for those mothers and infants.
