儿童大骨节病症状体征在临床诊断与X线诊断病例中的对比分析

目的了解儿童大骨节病症状体征在临床诊断和X线诊断病例中的分布特点,进行对比分析,为全面诊断儿童大骨节病提供一定的理论依据。方法采取整群随机抽样的方法抽取大骨节病病区2006名6～13岁儿童进行研究。结果两类诊断病例数存在统计学差异（χ2=67.052,P=0.001）,两类诊断患病率结果在性别间无显著性差异（χ2=0.147,P=0.702;χ2=1.501,P=0.220）,但患病率均有随年龄增加的趋势。单因素和多因素分析显示症状体征在两类诊断病例中分布不同。结论儿童大骨节病症状体征在临床诊断与X线诊断病例中的分布存在差异,临床应用时应两类诊断指标综合考虑。

Analysis of diagnostic criteria in children with Kaschin-Beck disease

Objective To understand the distributed feature of clinical symptoms and physical signs of Kaschin-Beck disease by epidemiological methods for children aged from 6 to 13 years old in order to provide scientific evidence of the early diagnosis,early prevention and early treatment.Methods 2 006 subjects from endemic areas of Shaanxi province in China were studied.Results The case number which was diagnosed was different by two methods（χ2=67.052,P=0.001）.The prevalence wasn＇t significant between clinical and X-ray diagnostic with gender（χ2=0.147,P=0.702;χ2=1.501,P=0.220）.The results showed that the prevalence rate of adolescent KBD could be increased with age.The single and multiple factor logistic regression analysis showed that the distribution of clinical symptoms and physical signs of KBD was different between clinical and X-ray diagnostic.Conclusion The distribution of clinical symptoms and physical signs of KBD was different between clinical and X-ray diagnostic.
