高压氧综合治疗脑外伤后持续性植物状态102例疗效分析

摘    要:目的探讨高压氧综合治疗脑外伤后持续性植物状态的疗效以及影响因素。方法回顾性分析2003/2007年采用高压氧综合治疗的102例创伤性植物状态病例,主要研究患者Glasgow昏迷量表评分、高压氧治疗次数以及高压氧治疗时机等因素与疗效的关系。高压氧治疗压力为0.2MPa,吸纯氧80min,1/d,20次为1疗程。结果总有效率为90.2%,意识恢复率63.7%。高压氧治疗开始时间≤30天及31~60天组有效率明显高于〉60天组(0.05〉P〈0.001)。结论高压氧综合治疗是治疗脑外伤后植物状态的一种有效方法,疗效与脑外伤时昏迷程度呈反比,与高压氧治疗开始时间,疗程呈正比。

Effect of Hyperbaric Oxygen Combined Therapy on Persistent Vegetative State Following Cerebral Trauma in 102 Patients

Abstract:Objective To study the efficacy and influential factors of hyperbaric oxygen combined therapy on persistent vegetative state following cerebral trauma. Methods By using Glasgow Coma Scale and studying the treatment frequency and treatment juncture of hyperbaric oxygen combined therapy, we retrospectively analyzed 102 patients in persistent vegetative state following cerebral trauma from March 2003 to March 2007. The pressure for HBO was 0. 2 MPa. The patients were subjected to 99. 2%-99. 7% oxygen for 80 rain with an interval of 10 min in the middle, once a day, twenty times as a course of treatment. Results The total effective rate was 90. 20% (92/102). Sixty-five patients regained consciousness (63.7%). The patients who started hyperbaric oxygenation in less than 30 days or in 30-60 clays after falling into a coma, recovered better than those receiving treatment in more than 60 days after coma (P〈0. 05 and P〈0. 001). Conclusion Hyperbaric oxygen combined therapy is effective for persistent vegetative state following cerebral trauma. Its efficacy is related with Glasgow coma scale score, the beginning time of treatment and the treatment period.
