微型种植体骨性支抗在临床的应用研究

摘    要:目的]探讨微型自攻型钛种植体作为骨性支抗在临床正畸工作中的应用及其优、劣势.方法]20例患者应用48颗微型种植体,用于拉前牙向后,关闭拔牙间隙时植入第2前磨牙与第1磨牙之间牙槽间隔,单侧磨牙缺失时植入第2磨牙的远中,需直立后牙者置于该牙齿的倾斜方向反向颊侧.微型种植体植入后可即刻加力,承受的牵动力为100~200 g.结果]安氏Ⅱ类骨性上颌突、双颌突病例12例治疗结束后,切牙切缘平均内收6.3~6.4 mm,支抗磨牙平均前移0.3~0.4mm,单侧恒磨牙缺失的病例磨牙支抗前移1.0~1.5 mm,面型均得到明显的改善.安氏I类、安氏Ⅲ类错(牙合)病例的治疗均达到理想效果.结论]自攻型微钛钉种植体作为骨性支抗具有接触面积小、稳定性强、力量强的优点;种植后即刻加力,缩短了疗程;正畸医师可以独立完成操作,患者也易于接受.

Abstract:Objective] To study the self-drilling titanium screw mini-implants as skeletal anchorage in the clinical application of orthodontic work, study its advantages and disadvantages. Methods] 20 patients treated with 48 micro-implant for anterior teeth pulled back. Implanted at the time extraction space closed second premolar and the first molar alveolar interval, when the unilateral molar implant distal second molar, to be placed upright in the posterior teeth by reversing the direction of tilt of the cheek teeth side. Micro-implants after immediate loading, will affect the force to withstand 100-200 g. Results ] Class Ⅱ skeletal maxillary protrusion, bimaxillary protrusion cases of 12 patients after treatment, the average margin of incisors adductor 6.3 ~ 6.4 mm, the average molar anchorage forward 0.3 ~ 0.4 mm, the missing cases of unilateral molar molar anchorage forward 1.0 ~ 1.5 mm, the surface shape were improved obviously. Class Ⅰ , Class Ⅲ malocclusion, are to achieve the desired effect. Conclusion] The seE-drilling miniscrew implants as skeletal anchorage with a small contact area, stability, strength and strong advantage; planted immediate loading, shorter course of treatment; orthodontic doctors can operate independently, patients are also easy to accept.
