神经肌肉放松训练对慢性闭角型青光眼患者心理和生活质量的影响

摘    要:目的探讨神经肌肉放松训练对慢性闭角型青光眼患者焦虑、抑郁状态和生活质量的影响。方法89例慢性闭角型青光眼患者分为观察组43例和对照组46例,对照组患者给予常规对症支持治疗和手术治疗,观察组患者在常规治疗和手术治疗的基础上进行神经肌肉放松训练。分别于训练前和训练3、6个月进行汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)和健康调查简表(SF-36)测定。结果2组患者训练前HAMA及HAMD总分比较差异均无统计学意义(P〉0.05);对照组患者训练3、6个月后HAMA和HAMD总分与训练前比较差异均无统计学意义(P〉0.05);观察组患者训练3、6个月后HAMA和HAMD总分显著低于训练前(P〈0.05)。训练3、6个月后观察组患者HAMA和HAMD总分显著低于对照组(P〈0.05)。2组患者训练前sF-36得分比较差异无统计学意义(P〉0.05)。观察组患者训练3、6个月后躯体疼痛(BP)、活力(VT)、情感角色(RE)、心理卫生(MH)和总的健康状况(GH)得分显著高于训练前(P〈0.05);观察组患者训练3、6个月后社会功能(SF)、躯体功能(PF)和躯体角色(RP)得分与训练前比较差异无统计学意义(P〉0.05)。对照组患者训练6个月后BP、sF和RE得分显著低于训练前(P〈0.05);对照组患者训练6个月后PF、RP、GH、VT和MH得分与训练前比较差异无统计学意义(P〉0.05)。训练3个月后观察组患者BP、GH、VT、SF、RE和MH得分显著高于对照组(P〈0.05);训练6个月后观察组患者sF-36量表得分显著高于对照组(P〈0.05)。结论神经肌肉放松训练可有效地改善慢性闭角型青光眼患者的焦虑、抑郁状态和生活质量。

Abstract:Objective To study the effect of progressive muscle relaxation training on anxiety, depression and quality of life in patients with chronic angle-closure glaucoma. Methods . A total of 89 patients with chronic angle-closure glaucoma were divided into observation group (n = 43 ) and control group( n =46 ), the patients in control group were treated with routine sup- portive treatment and operation, based on these, the patients in observation group were treated with progressive muscle relaxa- tion training. All the patients were evaluated with Hamilton anxiety scale (HAMA) , Hamilton depression scale (HAMD) and short form 36 health survey questionnaire (SF-36) before and 3,6 months after progressive muscle relaxation training. Results There was no significant difference in the HAMA, HAMD and SF-36 total scores before progressive muscle relaxation training between the two groups ( P 〉 0.05 ). There was no statistical difference in the total scores of HAMA and HAMD between pre- training and 3,6 months later. The total scores of HAMA and HAMD after training of 3 and 6 months were significantly lower than those before training in observation group ( P 〈 0.05 ). The total scores of HAMA and HAMD after training of 3 and 6 months in observation group were significantly lower than those in control group (P 〈0. 05 ). There was no significant difference in SF-36 score before training between the two groups (P 〉 0.05 ). The scores of bodily pain (BP) , vitality(VT) , role emotional ( RE), mental health ( MH ) and general health ( GH ) after 3,6 months training were significantly higher than those beforetraining in observation group ( P 〈 0.05 ). There was no significant difference in the scores of social function ( SF), physical function (PF) and role physical (RP) between pre-training and 3,6 months training later in observation group ( P 〉 0.05 ). The scores of BP, SF and RE after training of 3,6 months were significantly lower than those before training( P 〈 0.05 ). There was no significant difference in the scores of PF, RP, GH, VT and MH between pre-training and 3,6 months training later in control group ( P 〉 0.05 ). The scores of BP, GH, VT, SF, RE and MH after training of 3 months in observation group were significantly higher than those in control group(P 〈 0.05 ). After training of 6 months, the SF-36 score in observation group were significant- ly higher than those in control group( P 〈 0.05 ). Conclusion Progressive muscle relaxation training can effectively improve the anxiety, depression and quality of life in patients with chronic angle-closure glaucoma.
