烧伤整形术后感染相关因素及病原菌分析

目的探讨引起烧伤整形术后感染的因素及病原菌的种类，为预防烧伤患者整形术后的感染，促进烧伤患者整形术后的康复提供依据。方法回顾2005年10月～2010年10月731例烧伤行整形术的患者临床资料，并对感染因素和病原菌进行统计分析。结果731例烧伤行整形术患者感染78例，总感染率10．67％，其中术前创面情况、术前住院时间、手术时间、烧伤愈合时间与手术后感染明显相关；分泌物细菌培养75例阳性，感染病原菌以金黄葡萄球菌感染为主，占38．67％；大肠埃希菌占24．00％，假单胞铜绿杆菌感染占22．67％，变形杆菌感染占8．00％。结论手术前创面状况、术前住院时间、整形手术时间、烧伤愈合时间为烧伤整形术后感染主要影响因素。

Infective factors and pathogen analysis after burn surgery

Abstract] Objective To identify the factors for infection of burns after plastic surgery and species of bacteria responsi- ble in order to prevent infection of burns after plastic surgery and promote the recovery of burn patients after plastic sur- gery. Methods Clinical data on 731 patients at this hospital who underwent plastic surgery following a burn from Octo ber 2005 to October 2010 were reviewed. Factors for infection and data on the bacteria responsible were statistically ana lyzed. Results Of the 731 burn patients who underwent plastic surgery, 78 developed an infection. The overall rate of infection was 10.67%. The preoperative condition of the wound, the duration of hospitalization prior to surgery, the op- erating time, and the time the burn took to heal were significantly associated with infection. The bacteria responsible for infection were mostly Staphylococcus aureus （which accounted for 38. 67%）, Escherichia coli （which accounted for 24.00G）, Pseudomonas aeruginosa （which accounted for 22. 67%）, and Proteus rnirabilis （which accounted for 8.00%）. Conclusion Major factors for infection of burns after plastic surgery were the preoperative condition of the wound, the duration of hospitalization prior to surgery, operating time, and the time the burn took to heal. The main bacteria responsible for infection were S. aureus, E. coli, P. aeruginosa, and P. mirabilis.
