腹腔镜手术联合戈舍瑞林治疗子宫内膜异位症合并不孕症的临床研究

目的：探讨腹腔镜手术联合戈舍瑞林治疗子宫内膜异位症合并不育症的临床疗效。方法：回顾性分析2009年4月-2012年4月间腹腔镜手术后接受戈舍瑞林治疗的39例子宫内膜异位症合并不孕症患者（治疗组）和接受米非司酮治疗的30例患者（对照组）的临床资料。结果：所有手术均获成功；治疗组与对照组总有效率分别为87．18％和63．33％，二者比较，有统计学差异（χ2=21．24，P〈0．01）；治疗组的复发率（10．26％）显著低于对照组（36．67％，χ2=5．22，P〈0．05），治疗组的受孕率（58．97％）显著高于对照组（26．67％），差异有统计学意义（χ2=7．15，P〈0．01）。结论：腹腔镜手术联合戈舍瑞林治疗子宫内膜异位症合并不孕症疗效确切，可以降低复发率、提高受孕率，是子宫内膜异位症患者助孕治疗的重要选择。

Clinical Study of Patients with Endometriosis Complicated Infertility by Laparoscopic Operation Combined Goserelin

Objective： To treat the clinical efficacy of laparoscopic operation combined goserelin for treating endometri- osis complicated infertility. Methods：39 patients with endometriosis complicated infertility who underwent laparoscopic operation and were given the therapy of goserelin after the therapy（study group）, and 30 cases patients received the therapy of mifepristone, from April 2009 to April 2012, were retrospectively analysed. Results： All operations were per- formed successfully. The total effective rate in study group（87. 18%） was significantly higher than that in control group（63.33%）, there was significant difference between both groups（χ2 =21.24, P〈10. 01）. Patients in study group had significantly lower recurrence rate（10. 26%） than that in control group（36. 67%）, and significantly higher preg- nancy rate（58. 97%） than that in control group（26. 67%）, the difference was statistically significant（χ2 =5.22,χ2 = 7. 15, P〈0. 05 or P〈0. 01）. Conclusion： The therapy of laparoscopic operation combined goserelin is a effective meas- urement for endometriosis complicated infertility, which can significantly decrease the recurrence rate and increase the pregnancy rate and should be an important choice for patients with endometriosis to assist reproductive treatment.
