54例原发胃肠道非霍奇金淋巴瘤的临床及预后分析

１９８４年５月到１９９４年４月共收治成人原发胃肠道非霍奇金淋巴瘤（ＧＩ－ＮＨＬ）５４例，占同期成人总ＮＨＬ患者的１９．８％，同期原发结外淋巴瘤的４６．２％。经分期的５１例中，Ｉ期１７例，１１期２１例，ＩＩＩ期７例，ＩＶ期６例。经重新阅片病理分型的４６例中，低恶度的６例，中恶度２７例，高恶度的１３例。３０例在术前做过胃肠镜活检病理，２３例与术后病理一致，７例不一致。本组随诊率７５．９％，中位随访时间２０个月。多因素分析表明，诊断时疾病分期及治疗措施与生存期有显著相关。手术切除＋化疗或手术切除＋放、化疗患者的生存期明显长于单纯手术切除或单纯放疗和（或）化疗的患者。Ｓ期细胞比例也是影响预后的重要因素。年龄、性别、病理类型、病变部位及肿块大小与预后无显著相关性。

ENGLISH ABSTRACTS OF ORIGINAL ARTICLESPRIMARY GASTROINTESTINAL NONHODGK IN, SLYMPHOMA:A CLINICAL AND PROGNOSTICANALYSlS OF 54 CASES

iftyfour patients with prirnary gastrointestinalnon-Hodgkin's lymphoma (GINHL) adrnitted betweenMay 1984 and April 1994 were reviewed. They constituted 19. 8% of total adult NHL and 46. 2% of extranodal lymphoma patients in the same period. Out of 51staged cases, 17 were in stage I , 21 stage II, 7 stageIII and 6 stage IV at pesentation . Six patients were oflow grade, 27 intermediate grade and 13 high gradehistology according to the modified Working Formulation. Thirty cases underwent endoscopic biopsy, andthe pathologic diagnoses were in accordance with postoperative specimen in 23 and different in 7. Cox regression analysis indicated that survival correlatedmost strongly with stage of disease at presentation (P= 0. 0351 and treatment (P = 0. 0002). Survival wasrnuch longer in patients with complete surgical resection chernotherapy or resection chemotherapy radiotherapy than in those with resection or chemotherapy andlor radiotherapy alone. The percentage of S-phase cells was also a predicting factor for survival.Age . sex, histologic subtype , size or site of turnor didnot significantly correlated with prognosis.
