单纯性肥胖儿童生长发育与胰岛素抵抗、血脂水平的关系研究

目的 研究单纯性肥胖儿童生长发育与胰岛素抵抗、血脂水平的关系.方法 选取2013年1月至2015年7月本院收治的81例单纯性肥胖儿童为研究组,同期选取体检体质健康的81例儿童作为对照组;对两组儿童的糖化血红蛋白(HbAlc)、甲状腺激素T3、皮质醇(Cortisol)、血清瘦素(Leptin)、胰岛素(INS)、空腹血糖(FBG)、锌-0α2-糖蛋白(ZAG)、肥胖抑制素(Obestatin)、新厌食肽(Nesfatin-1)、胰岛素抵抗指数(Homa2-IR)、生长激素(GH)以及血脂水平进行检测对比分析.结果 两组比较发现,研究组ZAG、Cortisol、Leptin及T3水平明显高于对照组,而Obestatin、Nesfatin-1及GH水平明显低于对照组,组间比较差异均有统计学意义(均P＜ 0.05);研究组中INS、HbAlc、FBG以及Homa2-IR水平均明显高于对照组,差异均有统计学意义(均P＜ 0.05);而研究组TC、TG及LDL-C水平与对照组比较差异均有统计学意义(均P＜ 0.05).结论 单纯性肥胖儿童胰岛素抵抗、血脂水平均会发生明显的变化,各指标水平与肥胖具有相关性,应引起高度重视,做好有效的防治及监控工作.

The relationship between growth and development and insulin resistance,blood lipid level in simple obesity children

Objective To study the relationship between growth and development and insulin resistance,serum leptin,endocrine hormones,blood lipid levels in simple obesity children.Methods From January 2013 to July 2015,81 cases of simple obesity children treated in our hospital were selected as the study group,81 children with healthy physical examination were selected as the control group.The levels of HbAlc,T3,cortisol,leptin,INS,FBG,ZAG,obestatin,Nesfatin-1,Homa2-IR,GH,and blood lipid were measured and compared.Results The levels of ZAG,cortisol,leptin,and T3 in the study group were significantly higher than those in the control group,while the levels of obestatin,Nesfatin-1,and GH were significantly lower than those in the control group (P＜0.05).The levels of INS,HbAlc,FBG,and Homa2-IR in the study group were significantly higher than those in the control group (P＜0.05).The levels of TC,TG and LDL-C in the study group were significantly higher than those in the control group (P＜0.05).Conclusion Insulin resistance,serum leptin,endocrine hormones,and blood lipid levels in simple obesity children change obviously,which seriously affect the healthy growth and development of children,should be paid great attention to,with effective control and monitoring.
