高氧液治疗急性脑分水岭梗死的疗效观察

摘    要:目的 观察高氧液治疗急性脑分水岭梗死(Cerebral watershed infarction,CWI)的临床疗效.方法 选取本院86例急性脑分水岭梗死患者,随机分为对照组(43例)和治疗组(43例),对照组只给予改善循环、抗血小板聚集、营养神经等常规治疗,治疗组除常规治疗外,另给以高氧液500 ml静点,1次/d,持续2周.在治疗前、治疗后的第7、14天对两组患者进行神经功能缺损评分(NIHSS)、日常生活能力Barthel指数评分(BI)及治疗后6个月采用改良Rankin量表(MRS)测评比较两组疗效.结果 治疗后,治疗组的第7、14天NIHSS评分均较对照组明显减低(均P<0.05);治疗组BI比对照组显著升高(P<0.01);治疗后6个月治疗组MRS评分明显优于对照组(P<0.01),两组治疗过程中均未见明显不良反应.结论 高氧液治疗急性脑分水岭梗死疗效确切,能显著改善患者的神经功能缺损及预后,值得临床推广.

Abstract:Objective To observe the clinical efficacy of hyperoxia liquid in the treatment of acute cerebral watershed infarction.Methods 86 patients with acute cerebral watershed infarction treated at our hospital were selected and randomly divided into a control group (43 cases) and a treatment group (43 cases).The control group were given conventional treatment,such as improving circulation,anti-platelet aggregation,and nerve nutrition.The treatment group were given conventional treatment and 500 ml hyperoxia solution,ivgtt,qd,for 2 weeks.Before and day 7 and 14 after the treatment,both groups were scored by National Institute of Health stroke scale (NIHSS) and Daily life Barthel index score (BI);and 6 months after the treatment,both groups were evaluated by Modified Rankin scale (MRS).The curative effects were compared between these two groups.Results Day 7 and 14 after the treatment,the scores of NIHSS were significantly lower and the scores of BI significantly higher in the treatment group than in the control group (all P < 0.05) than the control group on the seventh and fourteenth day;Six months after the treatment,the MRS score was significantly better in the treatment group than in the control group (P < 0.01).No obvious adverse reactions were observed in the two groups during the treatment.Conclusions Hyperoxia liquid in the treatment of acute cerebral watershed infarction is effective and can improve the patients' nerve function defect and prognosis,so it is worth being clinically generalized.
