完全电视胸腔镜手术诊治24例原发性纵隔肿瘤的临床分析

目的 研究分析完全电视胸腔镜手术治疗原发性纵隔肿瘤的临床应用价值及临床疗效.方法 采用回顾性病例对照研究方法,选取收住本院的原发性纵隔肿瘤患者24例,肿瘤最大径线值1.5～13.0cm,平均(6.0±1.7) cm,均采用完全电视胸腔镜手术治疗.对围手术期各项指标进行观察记录.结果24例患者均顺利完成手术.平均手术时间为(90.3±19.6) min,平均术中出血量为(104.6±21.4)ml,平均术后住院时间为(7.9±3.2)d;围手术期未见死亡病例.依据平均肿瘤最大径线为界值,将患者分为体积较大组(＞6 cm)和体积较小组(≤6 cm),其中较大组手术时间和术中出血量均显著高于较小组(均P＜ 0.05),但两组术后住院时间比较差异无统计学意义(P＞0.05).结论 完全电视胸腔镜可用于各类原发性纵隔肿瘤的治疗,均可获得较好的临床疗效,且降低手术创伤.

Clinical analysis of 24 cases of primary mediastinal tumors treated by complete video-assisted thoracoscopic surgery

Objective To investigate the clinical effect of complete video-assisted thoracoscopic surgery in primary mediastinal tumors.Methods 24 cases of primary mcdiastinal tumors in our hospital were selected.The maximum diameter of the tumor was 1.5-13.0 cm,with an average of (6.0± 1.7) cm,and all of them were treated by complete video-assisted thoracoscopic surgery.Indexes during the perioperative period were observed and recorded.Results The surgery of 24 patients were successfully completed,the operation time was (90.3±19.6) min,intraoperative blood loss was (104.6±21.4) ml,postoperative hospital stay was (7.9±3.2)d,no death during treatment.The patients were divided into large size group (＞6 cm) and small size group (＜6 cm) according to the maximum tumor diameter.The operation time and the amount of bleeding in the large size group were significantly higher than those in the small size group (P＜0.05),but there was no statistically significant difference in postoperative hospitalization between the two groups (P＞0.05).Conclusion Complete video-assisted thoracoscopic surgery can obtain better clinical curative effect and reduce surgical trauma for the treatment of primary mediastinal tumors.
