摘    要:目的 研究限制性液体复苏在大失血致休克患者急诊救治中的应用.方法 选取2013年6月至2016年8月本院收治的84例失血性休克患者为研究对象,采用随机数表法将纳入患者分为观察组和对照组,每组42例.观察组给予限制性液体复苏,对照组给予传统液体复苏,比较两组临床疗效与复苏后实验室指标.结果 观察组多器官功能障碍综合征发生率、急性呼吸窘迫综合征发生率、病死率分别为2.4%、4.8%、9.5%,显著低于对照组的14.3%、19.0%、26.2% (P<0.05).观察组血小板计数、红细胞压积分别为(176.73±92.05)×109、(25.48±7.26)%,显著高于对照组的(114.59±96.32)×109、(19.33±4.35)%(P<0.05).结论 限制性液体复苏可以避免输液过量造成的机体内环境紊乱,为后续治疗创造良好的条件.

Abstract:Objective To study the application of limited flmd resuscitation in emergency treatment for patients with hemorrhagic shock.Methods From June,2013 to August,2016,84 patients with hemorrhagic shock treated at our hospital were taken as clinical research objects and were randomly divided into an observation group (42 cases) and a control group (42 cases).The observation group were given limited fluid resuscitation and the control group conventional fluid resuscitation;after resuscitation,the clinical effects and laboratory indicators after resuscitation in the two groups were compared.Results The incidences of multiple organ dysfunction syndrome and acute respiratory distress syndrome and mortality were 2.4%,4.8%,and 9.5% in the observation group,which were obviously lower than those in the control group (14.3%,19.0% and 26.2%) (all P < 0.05).The platelet count and hematokrit were (176.73±92.05) ×l09 and (25.48±7.26) % in the observation group,which were obviously higher than those in the control group[(114.59±96.32) ×l09 and (19.33±4.35) %] (both P < 0.05).Conclusions Limited fluid resuscitation can avoid disturbance in internal environment caused by excessive fluid infusion and create favorable conditions for subsequent treatment.
