进展期胃癌行腹腔镜辅助D2根治术患者术后发生并发症的相关因素分析

目的 探讨分析进展期胃癌行腹腔镜辅助D2根治术患者术后发生并发症的相关因素,为临床上防治腹腔镜辅助D2根治术的术后并发症提供参考.方法 回顾性分析2014年1月至2017年1月在本院行腹腔镜辅助D2根治术的116例进展期胃癌患者的临床资料,术后随访3～6a,统计术后并发症的发生情况,通过单因素分析和多因素回归分析(logistic回归模型)对患者术后并发症发生的相关因素进行分析总结.结果 116例患者的手术均却取得了成功.术后并发症总发生率为24.13％ (28/116),Ⅱ级及以上并发症发生率为12.07％(14/116).单因素分析结果显示年龄、术前合并症、术中失血量以及术前是否行辅助化疗与术后并发症有关(P＜0.05).Logistic回归分析结果显示年龄、术前合并症、术中出血量是术后并发症发生的独立危险因素(P＜0.05).结论 进展期胃癌患者在行腹腔镜辅助D2根治术时应注意对患者年龄、术前合并症以及术中出血量等多种术后并发症的相关因素进行评估和干预,降低术后并发症的发生率以保障患者预后.

Analysis of related factors of postoperative complications in patients with advanced gastric cancer undergoing laparoscopic assisted D2 radical resection

Objective To investigate and analyze the related factors of postoperative complications of laparoscopic assisted radical gastrectomy (D2) for advanced gastric cancer,so as to provide reference for the prevention and treatment of postoperative complications of laparoscopic assisted D2 radical resection.Methods The clinical data of 116 cases of advanced gastric cancer undergoing laparoscopic assisted D2 radical resection in our hospital from January 2014 to January 2017 was retrospectively analyzed.The postoperative complications were recorded during 3-6 years' follow up.The univariate analysis and multivariate regression analysis (logistic model) were used to analyze the related factors of postoperative complications.Results The operation of all the 116 patients were successful.The rate of postoperative complication was 24.13％ (28/116),the rate of grade Ⅱ and above complications was 12.07％ (14/116).Univariate analysis showed that age,preoperative complications,intraoperative blood loss,preoperative adjuvant chemotherapy were related to postoperative complications (P＜0.05);Logistic regression analysis showed that age,preoperative complications,postoperative blood loss were independent risk factors of postoperative complications (P＜0.05).Conclusion We should pay attention to the assessment and intervention of risk factors of postoperative complications such as age,preoperative complications,intraoperative blood loss during laparoscopic assisted D2 radical resection for advanced gastric cancer patients,to reduce the incidence of postoperative complications and guarantee the prognosis of the patients.
