妊娠并乙肝病毒感染患者的分娩方式意愿调查

目的 分析妊娠合并乙肝病毒感染患者分娩方式意愿调查,并分析对分娩结局的影响.方法 选取本院2013年9月至2015年3月收治妊娠并乙肝病毒感染患者为研究对象,对患者开展问卷调查,分析分娩方式意愿,并分析孕妇自愿选择分娩方式原因.结果 孕妇共285例,69例优先选择自然分娩,其中10例存在剖宫产指征;216例优先选择剖宫产,98例存在剖宫产指征.产前意愿是自然分娩产妇,实际阴道分娩比例占59.4％,显著高于剖宫产意愿产妇实际阴道分娩产妇比例22.2％,差异有统计学意义(P＜0.05).195例孕妇能够基本认识自然分娩的利弊,知识基本来自书本、朋友和医务人员,234例孕妇表示自己需要接受医务人员的指导.孕妇最关注的问题是母婴传播可能性,其次是安全性.196例剖宫产孕妇中,108例存在剖宫产指征,头盘不称、瘢痕妊娠以及骨盆异常占前3位.结论 孕妇产前分娩意愿存在很大差异,建议加强产前宣传指导教育工作,澄清分娩上的认识,降低剖宫产率.

Delivery method choices of pregnant women with hepatitis B virus infection

Objective To investigate the delivery method choices of pregnant women with hepatitis B virus infection and to analyze the influences on the delivery outcomes.Methods The pregnant women with Hepatitis B virus infection admitted into our hospital from September,2013 to March,2015 were selected as study objects and were surveyed with questionaire.Their delivery method choices and why they chose those choices were analyzed.Results A total of 285 pregnant women were selected.69 chose natural delivery;among the 69,10 had caesarean section indications.216 selected caesarean sections and only 98 had caesarean section indications.59.4％ of the 69 women took vaginal delivery and only 22.2％ of the 216,with a statistical difference (P ＜ 0.05).195 women basically knew the advantages and disadvantages of natural delivery from books,friends,and medical staff.234 indicated that they needed guidance from medical staff.Mother-infant transmission was the most concemed problem of the women and safety came next.Among the 196 taking caesarean section,108 had caesarean section indications (appetizers asymmetry,and scar pregnancy and pelvic abnormalities took the top three).Conclusion There are great differences in delivery method choices in the pregnant women before delivery.Prenatal education should be done well to make the pregant women know natural delivery well,so as to decrease the cesarean section rate.
