摘    要:目的 探究三维时间飞跃法磁共振血管成像(3D-TOF-MRA)结合多平面重建在血管压迫性三叉神经痛术前评估中的诊断价值.方法 选择2015年2月至2016年12月本院收治的血管压迫性三叉神经痛患者194例,均行三叉神微血管加压术(MVD)治疗.所有患者术前均予以3D-TOF-MRA检查结合多平面重建,将其检查结果与MVD术中对神经血管压迫(NVC)的检查结果对比,并分析3D-TOF-MRA术前显示三叉神经责任血管性质与MVD术中判定结果的一致性.结果 3D-TOF-MRA术前结合多平面重建对神经血管压迫的检查结果与MVD术中判定结果比较,差异无统计学意义(P>0.05);3D-TOF-MRA术前显示三叉神经责任血管性质与MVD术中判定结果具有较高的一致性(K=0.93).结论 3D-TOF-MRA结合多平面重建能清晰显示血管与三叉神经之间的关系,准确评估血管压迫性三叉神经痛患者的责任血管性质和神经血管压迫征象,能为临床制定MVD手术方案提供可靠的参考依据.

Abstract:Objective To investigate the diagnostic value of three-dimensional time-of-flight magnetic resonance angiography (3D-TOF-MRA) combined with multiplanar reconstruction in the preoperative evaluation of vascular compression trigeminal neuralgia.Methods 194 patients with vascular compression trigeminal neuralgia admitted in our hospital from February 2015 to December 2016 were treated with trigeminal micro vessel compression surgery (MVD).All the patients were given 3D-TOF-MRA examination combined with multiplanar reconstruction,compared the examination results and the test results of neurovascular compression (NVC) during MVD resection,and analyzed the consistency of 3D-TOF-MRA preoperative display results of trigeminal nerve vascular properties and during MVD.Results There were no statistically significant difference between preoperative neurovascular compression test results of 3D-TOF-MRA combined with multiplanar reconstruction and MVD operation results (P>0.05);preoperative 3D-TOF-MRA showed consistent determination results of trigeminal nerve responsible vascular properties with MVD surgery (K=0.93).Conclusions 3D-TOF-MRA combined with multiplanar reconstruction can clearly show the relationship between trigeminal nerve and blood vessel,accurately assess the nature of offending vessels and neurovascular compression features of patients with vascular compression trigeminal neuralgia,which can provide a reliable basis for the clinical development of MVD operation plan.
