大腿丛状神经鞘瘤1例

目的 探讨大腿根部丛状神经鞘瘤的临床及病理特点.方法 报告1例大腿丛状神经鞘瘤并进行相关文献复习.结果 63岁女性患者,因发现左大腿根部肿物2年余入院,入院后完善相关检查,行手术切除肿物.组织病理检查示真皮层深部多结节状肿物,每个结节均有完整的纤维包膜,结节主要由许多梭形细胞组成,多呈短束状,可见verocay小体,偶见核分裂像,结节间多为疏松的纤维性间质,免疫组化示S-100蛋白阳性,而SMA、NF及Ki-67均阴性,病理诊断:丛状神经鞘瘤.结论 丛状神经鞘瘤是一种较少见的外周神经肿瘤,在临床工作中根据其典型病理表现,注意与丛状神经纤维瘤等疾病鉴别.

A case of plexiform schwannoma in top-thigh

Objective To investigate the clinical and pathological features of plexiform schwannoma in top-thigh.Methods A case of plexiform schwannoma in top-thigh was reported and the related literatures were reviewed.Results The patient (female,63 years old) came to hospital because a mass was found in the top of her left thigh.After being hospitalized,routine laboratory tests were taken and the mass was removed completely.Pathological examination showed a multinodular mass in the deep dermis and every nodule had complete envelope;besides there was loose fibrous stroma between nodules.The nodule was composed of spindle cells in which verocay corpuscles and mitotic figures could be seen occasionally.Immunohistochemistry showed that S-100 protein was positive,while SMA,NK,and Ki-67 negative.Finally,the pathologic diagnosis was plexiform schwannoma.Conclusions Plexiform schwannoma is a kind of rare peripheral nerve tumor,which has special pathological characteristics.Plexiform schwannoma should be identified from plexiform neurofibroma during our clinical work.
