CT引导下靶点射频联合盘内小剂量胶原酶注射治疗颈椎间盘突出症的临床应用

摘    要:目的 探讨CT引导下靶点射频联合盘内小剂量胶原酶注射在治疗颈椎间盘突出症中的临床疗效,并评定此种方法的优越性及临床应用价值.方法 选取颈椎间盘突出症患者108例,随机分为以靶点射频热凝治疗的射频组及以靶点射频热凝联合盘内小剂量胶原酶注射治疗的联合组,按照颈椎病JOA标准计算并记录治疗前与治疗后1周、1个月、3个月、6个月JOA评分,并对两组患者的临床治疗效果进行比较分析.结果 治疗后1周、1个月、3个月、6个月时JOA评分均较术前显著改善,联合组较射频组疼痛评分显著提高.治疗后6个月,射频组的优良率为83.3%(45/54),联合组的优良率为90.7%(49/54).结论 CT引导下靶点射频联合盘内小剂量胶原酶注射治疗颈椎间盘突出症提高了射频热凝的准确性和成功率,其疗效明显优于单一微创治疗,值得临床推广应用.

Abstract:Objective To explore the therapeutic effect of target radiofrequency combined with the injection of a small dose ofcollagenase into the disc for cerival disc herniation.Methods One hundred and eight patients with cervical intervertebral disc herniation were randomly divided into a treatment group and a control group.The treatment group were treated with radiofrequency ablation and a small dose of collagenase.The control group were treated with collagenase chemolysis.The JOA scores before and 1 week and 1,3,and 6 months after the treatment were recorded.The treatment effects were compared between these two groups.Results The JOA scores 1 week and 1,3,and 6 months after the treatment were better than those before the treatment in both groups and were better in the treatment group than in the control group.The good and excellent rate was higher in the treatment group than in the control group (90.7% vs.83.3%).Conclusion CT-guided targeted radiofrequency combined with injection of a small dose of collagenase in treatment of cervical disc herniation can increase the accuracy and successful rate of radiofrequency and is more effective than only minimally invasive treatment,so it is worth being clinically generalized.
