（131）I治疗青少年甲状腺功能亢进症的疗效观察

目的观察131I治疗青少年甲状腺功能亢进症（甲亢）的疗效,探讨其治疗的可行性。方法对我院收集的120例青少年甲状腺功能亢进症患者进行131I治疗,治疗1年内定期复查肝功能、甲状腺功能、摄131I率及甲状腺显像（可计算甲状腺重量）,综合评价其治疗效果。结果 120例患者治疗前后肝功能指标变化比较,差异无统计学意义（P〉0.05）;治疗前后甲状腺功能指标、24 h摄131I率、甲状腺重量比较,差异均有统计学意义（P〈0.05或P〈0.01）。治疗后治愈80例（66.7%）,好转27例（22.5%）,发生甲状腺和能减退症（甲低）13例（10.8%）。一次给药与重复给药治疗治愈率比较,差异有统计学意义（53.4%vs 71.9%,P〈0.05）,但对甲低发生率影响不明显（10.2%vs 12.5%,P〉0.05）。结论131I治疗青少年甲状腺功能亢进症疗效安全有效,但治疗时仍需慎重,以防甲低的发生。

Clinical observation of 131I in the treatment of adolescent patients with hyperthyroidism

Objective To observe the effect of 131I in the treatment of adolescent patients with hyperthyroidism,and explore the feasibility of treatment.Methods 120 cases of adolescent patients with hyperthyroidism were treated with 131I.Liver function,thyroid function,the rate of taking 131I at 24 h and thyroid image(counted the thyroid weight) were checked in a year.Results Among 120 cases of patients with hyperthyroidism,the liver function had no significant difference between before and after treatment(P>0.05);the index of thyroid function,the rate of taking 131I at 24 h and thyroid weight were significant differences between before and after treatment(P<0.05 or P<0.01).After treatment 80 cases(66.7%) were cured,27 cases(22.5%) were improved,13 cases(10.8%) suffered with hypothyroidism.The cured rate was significant difference between single administration and repeat administration(53.4% vs 71.9%,P<0.05),but the incidence of hypothyroidism was not significant difference(10.2% vs 12.5%,P>0.05).Conclusion 131I treatment is safe and effective in the treatment of adolescent patients with hyperthyroidism,but the treatment need take care,in order to aviode the occurrence of hypothyroidism.
