血液透析联合血液灌流治疗尿毒症性皮肤瘙痒的临床观察

目的观察血液透析联合血液灌流治疗尿毒症性皮肤瘙痒的临床效果及不良反应情况,为临床诊治方案提供参考依据。方法将已确诊的50例尿毒症性皮肤瘙痒患者随机分为研究组和对照组,每组各25例。对照组给予单纯血液透析治疗;研究组在对照组基础上加用血液灌流治疗。应用统计学软件对比分析两组患者治疗8周后的总有效率、甲状旁腺激素（PTH）、β2微球蛋白（β2MG）水平变化及不良反应情况。结果研究组的总有效率明显高于对照组,差异有统计学意义（P〈0.05）。治疗8周后,研究组PTH、β2MG水平明显低于治疗前,且研究组PTH、β2MG水平明显低于对照组（P〈0.05）。结论血液透析联合血液灌流治疗尿毒症性皮肤瘙痒疗效好、作用快、不良反应少,是目前治疗尿毒症性皮肤瘙痒的一种较佳治疗方案,值得临床推广应用。

Clinical observation of hemodialysis combined with hemoperfusion in the treatment of uremic pruritus

Objective To observe the clinical effect and adverse reaction of hemodialysis combined with hemoperfusion in the treatment of uremic pruritus,in order to provide reference for clinical diagnosis and treatment program.Methods 50 cases of patients who were diagnosed with uremic pruritus were randomly divided into study group and control group,each groups had 25 patients.The control group was given with purely hemodialysis treatment;on the basis of control group,the study group was treated combined with hemoperfusion.The total effective rate,PTH,β2MG level and adverse reactions were compared and analyzed between the two groups by statistical software after 8 weeks treatment.Results The total efficiency of the study group was significantly higher than that of the control group,the difference was statistically significant(P<0.05).After 8 weeks treatment,the PTH,β2MG level of the study group were significantly lower than those of before treatment,which were also lower than those of the control group(P<0.05).Conclusion Hemoperfusion combined with hemodialysis in the treatment of uremic pruritus is effective,fast-acting and has lower adverse reactions,which is the better treatment plan for uremic pruritus.It is worthy of clinical application.
