外伤性脑内血肿的显微外科治疗

目的 探讨显微手术在外伤性脑内血肿患者中的应用.方法 对2010年1月~2011年4月在显微镜下手术治疗的28例外伤性脑内血肿患者临床资料进行回顾性分析.结果 术后1~3 d复查头颅CT示,血肿清除大于80%者23例,血肿清除小于80%者5例.随访3~15个月,按GOS评估预后,恢复良好的15例,中残6例,重残3例,植物生存l例,死亡3例.结论 侧裂区、矢状窦区、大脑深部及小脑等部位的外伤性脑内血肿适合于显微手术治疗.

Microsurgery for traumatic intracerebral hematomas (report of 28 cases)

Objective To explore the application of microsurgery to the traumatic intracerebral hematomas. Method The clinical data of 28 patients with traumatic intracerebral hematomas, who were treated with microsurgery from January, 2010 to April, 2011, were analyzed retrospectively. Results CT scan from 1 to 3 days after the surgery showed that of 28 patients with traumatic intracerebral hematomas, 23 received removal of more than 90% volume of hematomas and 5 less than 80%. According to GOS, of 28 patients, 15 were recovered well, 6 moderately disabled, 3 severe disabled, 1 vegetatively survived and 3 died. Conclusion The traumatic intracerebral hematomas in the lateral cerebral fissure area, sagittal sinus area, deep cerebral area and cerebellum should be removed by microsurgery.
