急性脑梗死部位与血糖的相关性探讨

摘    要:目的探讨急性脑梗死部位与血糖的相关性及可能的发病机制。方法连续登记2010年9月~2011年9月入住我院的急性脑梗死患者188例的临床资料,并按急性卒中影像学DWI分型将急性脑梗死进行分型并分析与血糖的关系。结果在纳入分析的188例患者中,单发梗死病灶68例(36.2%),其中皮层梗死4例(2.1%)、皮层-皮层下梗死20例(10.6%)、大穿通动脉梗死28例(14.9%)、小穿通动脉梗死16例(8.5%);多发梗死病灶120例(63.8%),其中单侧前循环多发梗死44例(23.4%)、双侧前循环多发梗死12例(6.4%)、后循环多发梗死40例(21.3%)、前-后循环多发梗死24例(12.8%)。其中原有糖尿病史36例(19.1%),新发现糖尿病14例(7.5%),糖调节受损21例(11.4%)。采用SPSS13.0统计软件,各亚型与高血糖的关联分析χ2=14.83,P=0.020,r=0.152。各亚型糖代谢异常频率差异的比较χ2=17.79,P=0.000。各亚型糖化血红蛋白异常频率差异比较χ2=35.57,P=0.000。结论不同部位的急性脑梗死与血糖水平有关联性,对具有高危因素的人群或已经患有脑梗死的患者在预防和治疗上应遵循个体化原则。

Relationships between acute cerebral infarction position and glucose in brain ischemia

Abstract:Objective To explore the relationships between acute cerebral infarction position and glucose.Methods The clinical data of patients with acute cerebral infarction were analyzed retrospectively.Their diagnoses were established by imaging and other examinations.The subtypes of patients were identified according to image classification in acute stroke,and their major risk factors were investigated and analyzed.Results In the 188 cases,68 cases(36.2%) with trends lesions,among of them,4 patients(2.1%)with cortex infarction,20 cases(10.6%) with cortex-subcortical infarction,28 cases(14.9%)with big wear the artery infarction,16 cases(8.5%)with small wear the artery infarction.120 cases(63.8%)with multiple lesions,among of them,44 cases(23.4%)with multiple infarcts of anterior,12 cases(6.4%)with multiple infarcts of bilateral procyclic,40 with posterior circulation infarction,24 cases(12.8%)with anterior-posterior circulation infarction.Thirty-six patients(19.1%)had diabetes history,another fourteen(7.5%)were newly diagnosed within this study,and tweenty-one(11.4%)were found glycoregulation abnormality.Used SPSS 13.0,association analysis between stroke subtypes and glucose:χ2=14.83,P=0.020,r=0.152.Conclusion Different acute cerebral infarction position could be associated with glucose,which may indicate us to improve the individual therapy in brain ischemia.
