欣普贝生用于足月胎膜早破病例引产的临床研究

目的 探讨欣普贝生用于足月胎膜早破引产病例的临床疗效及安全性.方法 将80例足月胎膜早破≥12h、Bishop宫颈评分≤6分、无妊娠合并症、无阴道分娩禁忌证、单胎头位的初产妇随机分为两组,分为阴道放置欣普贝生的研究组和小剂量缩宫素静滴的对照组,每组40例.观察比较两组用药12h后宫颈Bishop评分变化、临产情况、分娩方式、产后2h出血量、产程及新生儿的情况.结果 研究组促宫颈成熟评分及引产成功率均高于缩宫素组,差异有统计学意义(P<0.05),用药后至临产的时间两组比较差异有统计学意义(P<0.05),研究组的剖宫产率低于对照组,两组的产后出血量及胎儿窘迫发生率、新生儿窒息率无统计学意义(P>0.05).结论 欣普贝生用于足月胎膜早破安全有效,与缩宫素相比,促子宫颈成熟有效率高,临产时间短,对新生儿不良影响小,可有效降低难产率及剖宫产率.

The clinical study of dinoprostone in induction of premature rupture of the membrane pregnancies in term

Objective To investigate the clinical efficiency and safety of dinoprostone in induction of premature rupture of the membrane pregnancies in term.Methods Eighty primiparas who were full term premature rupture of membrane≥12h,cervical Bishop score≤6,without pregnant complications,without vaginal delivery contraindications,single pregnancy,head position were randomly divided into observation group(one piece of dinoprostone was put into the vaginal) and control group(low-dose intravenous infusion of oxytocin),forty cases respectively.The changes of cervical Bishop score,labor conditions and modus of delivery,postpartum bleeding in two hours,labor and the situation of neonates in the two groups were compared.Results The success rates of observation group cervical ripening Bishop score and labor induction were higher than the control group,there was significant difference(P<0.05).There was significant difference between the two groups the time intervals from administration to labor(P<0.05).The cesarean section rate of observation group lower than control group.There was no significant difference in the two groups of postpartum hemorrhage and the incidence of fetal distress,neonatal asphyxia(P>0.05).Conclusion Compared with oxytocin,dinoprostone can be used for induction of premature rupture of the membrane pregnancy in term effectively and safely,promote the effective rate of cervical maturation,shorten the labor time,little harmful effects on neonates and effectively reduce the rate of dystocia and cesarean section.
