角膜裂伤修补联合白内障摘出Ⅰ期人工晶状体植入术

目的评价角膜裂伤修补联合白内障摘出Ⅰ期人工晶状体植入手术的效果。方法62例(62眼)进行联合手术,手术中使用玻璃酸钠形成前房,复位虹膜、维持眼压,保护眼内组织。所有病例均进行Ⅰ期人工晶状体植入术,其中囊袋内植入52眼,睫状沟植入6眼,缝线固定4眼。结果62眼均顺利完成手术。术后视力均有不同程度提高,最佳矫正视力≥0.3者48眼,占77.42%;低于0.3者14眼,占22.58%。影响视力恢复的主要原因是眼外伤所致的角膜中央区瘢痕、角膜不规则散光和外伤性黄斑变性。结论联合手术安全性高,视力恢复好,并发症少。手术中应用玻璃酸钠有助于维持前房,保护眼内组织,安全有效的进行手术和处理并发症。

Microsurgical repair of traumatic corneal laceration combined with primary cataract extraction and intraocular lens implantation

Objective To evaluate the clinical results of the microsurgical repair of traumatic corneal laceration combined with primary cataract extraction and intraocular lens implantation. Methods 62 eyes of 62 patients were performed with combined surgery. We used sodium hyaluronate to form anterior chamber, restore iris, maintain intraocular pressure and protect tissues in eyes. Intraocular lens implantation was performed in all eyes, 52 lens were implanted in bags, 6 lens were implanted in iridociliary sulcus and 4 lens were fixated by suture. Visual outcomes and complications were evaluated. Results The surgery was finished successfully in 62 eyes. The visual acuity increased in all cases. 48 eyes achieved the best correct visual acuity of 0.3 or better. 14 eyes were less than 0.3. Major causes of limited visual acuity were central corneal scar, irregular astigmatism and traumatic maculopathy. Conclusions Combined surgery is associated with favorable visual outcome and a low rate of postoperative complications. The application of sodium hyaluronate in combined surgery contribute to protect tissues in eyes and effectively deal with other traumatic complications.
