胰腺节段性切除治疗胰腺头颈部疾病的评价

目的 探讨胰腺节段性切除术治疗胰腺头颈部良性占位性病变的可行性和实用性。方法对2004年-2008年收治的15例胰腺头预部良性占位性病变的患者，实施了节段性胰腺切除，对该部位病变手术方式选择、手术步骤、手术要点、术后并发症和手术对患者胰腺内外分泌功能的影响进行分析。结果15例患者被实施了不同侧重选择的节段性胰腺切除术，其中胰腺颈部和部分胰头切除术6例，胰腺颈部切除术5例，胰颈和部分胰体部切除术4例。肿瘤病理分别是粘液性囊腺瘤、浆液性囊腺瘤、单纯囊肿、神经内分泌肿瘤和导管内乳头状粘液瘤。手术并发症发生率为20％，无手术死产，患者术后1周，血糖值与术前比较差异无统计学意义，术后检查粪便无脂肪微粒。结论节段性胰腺切除是胰腺头预部良性占位性病变较好的手术方式。
